Overview of the medicine
Varda 10 mg Tablet contains Vardenafil, a phosphodiesterase type 5 (PDE5) inhibitor, used to treat erectile dysfunction (impotence) in men. It helps to achieve and maintain an erection sufficient for sexual activity.
Uses & Indications
Dosage
Adults
Recommended starting dose is 10 mg, taken orally approximately 25 to 60 minutes before sexual activity. Dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. Maximum frequency is once daily.
Elderly
Consider a starting dose of 5 mg in patients ≥65 years due to increased exposure.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Not studied in severe renal impairment or end-stage renal disease.
How to Take
Take Varda 10 mg tablet orally with or without food, approximately 25-60 minutes before sexual activity. Do not take more than one dose per day.
Mechanism of Action
Vardenafil selectively inhibits phosphodiesterase type 5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum of the penis. When sexual stimulation causes local release of nitric oxide, PDE5 inhibition by vardenafil leads to increased cGMP levels, resulting in smooth muscle relaxation and inflow of blood into the penis, causing an erection.
Pharmacokinetics
Onset
Typically within 30-60 minutes after administration.
Excretion
Mainly excreted in the feces (approximately 91-95%) and to a lesser extent in the urine (approximately 2-6%) as metabolites.
Half life
Approximately 4-5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are typically reached within 0.5-2 hours (median 1 hour).
Metabolism
Primarily metabolized in the liver by cytochrome P450 (CYP) isoenzymes, mainly CYP3A4, and to a lesser extent CYP3A5 and CYP2C9.
Side Effects
Contraindications
- •Concomitant use of nitrates (e.g., nitroglycerin) in any form or nitric oxide donors.
- •Concomitant use of riociguat.
- •Hypersensitivity to vardenafil or any component of the tablet.
- •Patients with severe cardiovascular disease for whom sexual activity is inadvisable.
- •Recent history of stroke or myocardial infarction.
- •Known hereditary degenerative retinal disorders (e.g., retinitis pigmentosa).
- •Severe hepatic impairment (Child-Pugh C).
- •End-stage renal disease requiring dialysis.
Drug Interactions
Nitrates
Potentiation of hypotensive effects, potentially life-threatening. Concomitant use is contraindicated.
Riociguat
Concomitant use is contraindicated due to potentiation of hypotensive effects.
Alpha-blockers
Increased risk of symptomatic hypotension. Initiate vardenafil at a low dose with caution if alpha-blockers are used.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, erythromycin)
Increased vardenafil plasma levels. Dose adjustment may be necessary. Concomitant use with potent CYP3A4 inhibitors (e.g., ritonavir, indinavir) is contraindicated or requires significant dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as vardenafil is highly protein bound and not significantly excreted in urine.
Pregnancy & Lactation
Varda 10 mg is not indicated for use in women. No data available on its use in pregnant or lactating women.
