Overview of the medicine
Varda 20 mg Tablet contains Vardenafil, a phosphodiesterase-5 (PDE5) inhibitor, used to treat erectile dysfunction (impotence) in men. It works by increasing blood flow to the penis during sexual stimulation.
Uses & Indications
Dosage
Adults
The recommended starting dose is 10 mg taken orally, approximately 25 to 60 minutes before sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. Maximum frequency is once daily.
Elderly
No dose adjustment is required for elderly patients (≥65 years) based on age alone. However, consider starting with 5 mg in patients >65 years due to increased plasma concentrations.
Renal_impairment
No dose adjustment is required in patients with mild to moderate renal impairment. In severe renal impairment (CrCl <30 mL/min), initial dose of 5 mg should be considered, maximum 10 mg.
How to Take
Take orally with or without food, approximately 25 to 60 minutes before sexual activity. Sexual stimulation is required for Vardenafil to be effective.
Mechanism of Action
Vardenafil inhibits phosphodiesterase type 5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. By inhibiting PDE5, Vardenafil increases cGMP levels, leading to smooth muscle relaxation and increased blood flow into the penis, resulting in an erection upon sexual stimulation.

Onset
Onset of action is typically within 25-60 minutes.
Excretion
Excreted predominantly in the feces (approximately 91-95% of an oral dose) and to a lesser extent in the urine (approximately 2-6%).
Half life
Mean terminal half-life is approximately 4-5 hours.
Absorption
Rapidly absorbed. Mean absolute oral bioavailability is approximately 15%.
Metabolism
Primarily metabolized by hepatic cytochrome P450 (CYP3A4) and to a lesser extent by CYP2C9 and CYP2C8. The major circulating metabolite is M1, which is also active.
Side Effects
Contraindications
- •Hypersensitivity to vardenafil or any component of the tablet
- •Concomitant use with nitrates or nitric oxide donors (e.g., nitroglycerin, isosorbide dinitrate) due to the risk of severe hypotension
- •Concomitant use with guanylate cyclase stimulators (e.g., riociguat)
- •Patients with severe cardiovascular disease for whom sexual activity is inadvisable
- •Severe hepatic impairment (Child-Pugh C)
- •End-stage renal disease requiring dialysis
- •Recent stroke or myocardial infarction
- •Known hereditary degenerative retinal disorders (e.g., retinitis pigmentosa)
- •Patients with uncontrolled hypotension (<90/50 mmHg) or hypertension (>170/110 mmHg)
- •Concomitant use with strong CYP3A4 inhibitors in certain patient groups (e.g., ketoconazole, itraconazole, ritonavir, indinavir in patients >75 years)
Drug Interactions
Riociguat
Concomitant use contraindicated due to risk of symptomatic hypotension.
Alpha-Blockers
May lead to symptomatic hypotension. Initiate Vardenafil at 5 mg, with at least 6 hours between doses of alpha-blocker and Vardenafil.
Other ED treatments
Safety and efficacy of combinations with other ED treatments not studied, not recommended.
Nitrates/Nitric Oxide Donors
Potentiation of hypotensive effects, potentially life-threatening. Concomitant use is contraindicated.
CYP3A4 Inhibitors (Ketoconazole, Itraconazole, Ritonavir, Indinavir, Erythromycin, Clarithromycin)
Increase vardenafil exposure. Dose adjustment required, potentially lower max dose or frequency.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In volunteer studies, doses up to 120 mg vardenafil were administered. The most common adverse event observed was back pain/myalgia and abnormal vision. Management should be supportive, as dialysis is not expected to accelerate clearance due to high protein binding.
Pregnancy & Lactation
Vardenafil is not indicated for use in women. No data are available on the use of vardenafil in pregnant or lactating women.
