Description
Overview of the medicine
Varizil 400 mg Tablet contains Aciclovir, an antiviral drug used to treat infections caused by herpes viruses, including herpes simplex (cold sores, genital herpes) and varicella zoster (chickenpox, shingles). It works by stopping the growth and spread of the virus in the body.
Uses & Indications
Dosage
Adults
Herpes Zoster (Shingles): 800 mg five times daily for 7 to 10 days. Varicella (Chickenpox): 800 mg four times daily for 5 days. Treatment of HSV: 200 mg five times daily for 5 days. Suppression of recurrent HSV: 400 mg twice daily.
Elderly
Careful dosage adjustment is advised in elderly patients, especially those with impaired renal function. Hydration should be maintained.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl 10-25 mL/min: 800 mg every 8 hours for Zoster; 200 mg every 8 hours for HSV. For CrCl <10 mL/min: 800 mg every 12 hours for Zoster; 200 mg every 12 hours for HSV.

Take Varizil 400 mg Tablet orally, with or without food. It is important to drink plenty of water while taking this medicine to prevent kidney problems. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Aciclovir is a synthetic purine nucleoside analogue. It is selectively taken up by herpesvirus-infected cells, where it is converted to aciclovir monophosphate by viral thymidine kinase. This is then further converted to aciclovir triphosphate, which inhibits viral DNA polymerase and is incorporated into viral DNA, leading to chain termination and inhibition of viral replication.
Pharmacokinetics
Onset
Variable, generally within 24-48 hours for symptomatic relief, but full antiviral effect may take longer.
Excretion
Mainly excreted unchanged in the urine by glomerular filtration and tubular secretion. Renal clearance is directly related to creatinine clearance.
Half life
Approximately 2.5 to 3.3 hours in adults with normal renal function. It is longer in patients with renal impairment.
Absorption
Poorly absorbed from the gastrointestinal tract; bioavailability is approximately 10-20%. Peak plasma concentrations are reached within 1.5 to 2 hours after oral administration.
Metabolism
Primarily metabolized to an inactive metabolite, 9-carboxymethoxymethylguanine, which accounts for approximately 10-15% of the dose.
Side Effects
Contraindications
- •Hypersensitivity to Aciclovir or Valaciclovir or any component of the formulation.
- •Severe allergic reactions to previous doses of Aciclovir.
Drug Interactions
Probenecid
Increases aciclovir plasma half-life and AUC by reducing renal tubular secretion. Concurrent use may require dosage adjustment.
Nephrotoxic drugs
Concurrent use with other nephrotoxic drugs may increase the risk of renal dysfunction. Close monitoring of renal function is recommended.
Mycophenolate Mofetil
Increases plasma AUC of both aciclovir and the inactive metabolite of mycophenolate mofetil. Monitor for signs of toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include headache, nausea, vomiting, dizziness, confusion, hallucinations, and crystalluria leading to renal failure. Management involves symptomatic and supportive care. Hemodialysis can effectively remove Aciclovir from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Aciclovir is excreted in breast milk; caution should be exercised when administered to a nursing mother.
