Overview of the medicine
Varizil 500 mg Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2 grams once daily (or in two divided doses) via IV or IM route, depending on the severity of the infection. For severe infections, up to 4 grams daily may be used. For uncomplicated gonorrhea, a single IM dose of 500 mg is common.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal and hepatic function. Dosage should be based on severity of infection and renal function.
Renal_impairment
No dosage adjustment is necessary for renal impairment if hepatic function is normal, up to a maximum daily dose of 2g. For severe renal and hepatic impairment, monitoring of plasma concentrations may be prudent.
How to Take
Varizil 500 mg Injection can be administered either intramuscularly (IM) or intravenously (IV). For IM administration, the powder should be reconstituted with Lidocaine 1% solution (for pain reduction). For IV administration, it should be reconstituted with sterile water for injection and administered slowly over 2-4 minutes, or as an infusion over 30 minutes.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bactericidal activity.

Onset
Within 1-3 hours for peak plasma concentration.
Excretion
Approximately 50-60% excreted unchanged in urine and 40-50% in bile/feces.
Half life
Approximately 6-9 hours.
Absorption
Rapidly and completely absorbed after IM administration; IV administration provides immediate 100% bioavailability.
Metabolism
Minimally metabolized; primarily by gut flora and non-enzymatic hydrolysis in the small intestine.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone or any other cephalosporins
- •History of severe hypersensitivity to any beta-lactam antibiotic
- •Neonates (especially premature) with hyperbilirubinemia, or those requiring calcium-containing IV solutions due to risk of precipitation.
Drug Interactions
Probenecid
May decrease renal clearance of ceftriaxone, increasing plasma levels.
Aminoglycosides
Synergistic effect, but should not be mixed in the same syringe/infusion.
Calcium-containing solutions
Contraindicated in neonates; in other age groups, administer sequentially with flushing.
Oral Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring closer INR monitoring.
Storage
Store below 30°C in a dry place, away from light. Reconstituted solution should be used immediately or stored for a short period as directed.
Overdose
In case of overdose, treatment should be symptomatic and supportive. Ceftriaxone is not significantly removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use in pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations in breast milk; caution should be exercised when administered to a nursing mother.
