Overview of the medicine
Vasocor 5 mg Tablet contains Nicorandil, a potent anti-anginal medication used to prevent and treat symptoms of stable angina pectoris. It works by dilating both arterial and venous blood vessels, improving blood flow to the heart.
Uses & Indications
Dosage
Adults
Initial dose 5 mg twice daily, increasing to 10-20 mg twice daily based on response. Maximum dose 40 mg daily.
Elderly
No specific dose adjustment needed, but caution advised. Start with lower dose.
Renal_impairment
No dose adjustment usually required.
How to Take
Vasocor tablets should be swallowed whole with water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Nicorandil acts as both a potassium channel activator and a nitrate. It activates ATP-sensitive potassium channels in vascular smooth muscle cells, leading to hyperpolarization and vasodilation of both arteries and veins. Additionally, its nitrate component provides nitric oxide (NO), further causing vasodilation and reducing myocardial preload and afterload.

Primarily excreted in the urine as metabolites.

Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Bioavailability is about 75%.
Metabolism
Primarily metabolized in the liver, mainly by denitration and subsequent metabolism.
Side Effects
Contraindications

Drug Interactions
NSAIDs
May reduce the hypotensive effect of Nicorandil.
Phosphodiesterase-5 inhibitors (e.g., Sildenafil, Tadalafil)
Concomitant use is contraindicated due to severe hypotensive effects.
Other vasodilators, antihypertensives, tricyclic antidepressants, alcohol
May enhance the hypotensive effect of Nicorandil.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypotension, tachycardia, and peripheral vasodilation. Treatment is symptomatic and supportive, including elevation of legs, fluid administration, and vasopressors if necessary.
Pregnancy & Lactation
Not recommended during pregnancy and lactation due to insufficient data. Use only if potential benefit outweighs risk, and under strict medical supervision.
