Overview of the medicine
Veagra 50 mg Tablet is used to treat erectile dysfunction (impotence) in men. It works by increasing blood flow to the penis to help men get and keep an erection when sexually aroused.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately 1 hour before sexual activity. Based on efficacy and toleration, the dose may be increased to 100 mg or decreased to 25 mg. The maximum recommended dosing frequency is once per day.
Elderly
No dosage adjustment is required for elderly patients. However, a lower starting dose (25 mg) may be considered due to potential age-related decline in renal/hepatic function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take the tablet orally with or without food, approximately 1 hour before sexual activity. Do not take more than one dose in 24 hours.
Mechanism of Action
Sildenafil, a PDE5 inhibitor, selectively inhibits cGMP-specific phosphodiesterase type 5 (PDE5). When sexual stimulation causes local release of nitric oxide, PDE5 inhibition by sildenafil causes increased levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penis.

Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of the administered oral dose) and to a lesser extent in the urine (approximately 13% of the administered oral dose).

Rapidly absorbed after oral administration, with absolute bioavailability of about 41%. Peak plasma concentrations are reached within 30-120 minutes (median 60 minutes).
Metabolism
Primarily metabolized by hepatic microsomal isoenzymes CYP3A4 (major route) and CYP2C9 (minor route). The major circulating metabolite, N-desmethyl sildenafil, has about 50% of the activity of the parent drug.
Side Effects
Contraindications
- Concomitant use of nitrates in any form (due to severe hypotensive effects).
- Concomitant use of guanylate cyclase stimulators (e.g., riociguat).
- Hypersensitivity to sildenafil or any component of the tablet.
- Patients with severe cardiovascular events (e.g., recent myocardial infarction or stroke).
- Severe hepatic impairment.
- Hypotension (<90/50 mmHg).
Drug Interactions
Alcohol
Excessive alcohol intake may increase the risk of orthostatic hypotension.
Nitrates
Absolute contraindication; potentiates hypotensive effects, leading to severe hypotension or even death.
Riociguat
Concomitant use is contraindicated due to increased risk of hypotension.
Alpha-blockers
May lead to symptomatic hypotension in some patients, especially within 4 hours of sildenafil administration.
CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole, Erythromycin, Cimetidine)
Increase sildenafil plasma concentrations, requiring lower initial doses of sildenafil.

Overdose
In studies with healthy volunteers of single doses up to 800 mg, adverse events were similar to those seen at lower doses but incidence rates were increased. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly protein bound and not eliminated in the urine.

Veagra is not indicated for use in women, including during pregnancy or lactation. Studies in animals have not shown direct or indirect harmful effects with respect to reproductive toxicity.
