Description
Overview of the medicine
Veelac (Lactulose) is a synthetic disaccharide used to treat chronic constipation and hepatic encephalopathy. It works as an osmotic laxative by drawing water into the colon, softening stools and increasing bowel movements. In hepatic encephalopathy, it reduces ammonia absorption by acidifying the colon content.
Uses & Indications
Dosage
Adults
For constipation: Initially 15-45 ml daily, adjusted according to response. For hepatic encephalopathy: Initially 30-45 ml two to three times daily, adjusted to produce 2-3 soft stools per day.
Elderly
Same as adult dosage, but caution with electrolyte balance for prolonged high doses.
Renal_impairment
No specific dose adjustment required due to minimal systemic absorption.
How to Take
Oral administration. Can be taken directly or mixed with water, fruit juice, or milk. For best results, take at the same time each day.
Mechanism of Action
Lactulose is metabolized by colonic bacteria into low molecular weight organic acids, which acidify the colon content and cause an osmotic effect. This increases stool water content and softens the stool. In hepatic encephalopathy, the acidic environment converts ammonia (NH3) to ammonium (NH4+), which is not readily absorbed, thus reducing blood ammonia levels.

Laxative effect: 24-48 hours. Hepatic encephalopathy: may take several days for full effect.

Primarily excreted unchanged in feces. Small amounts (less than 3%) excreted in urine.
