Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the prophylaxis and chronic treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
One 10 mg tablet daily in the evening.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment is required.
How to Take
Take orally, once daily in the evening, with or without food. For asthma, it should be taken in the evening. For allergic rhinitis, the time of administration may be individualized to suit patient needs. For both conditions, evening administration is generally preferred.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor, thereby inhibiting the actions of leukotrienes (LTC4, LTD4, LTE4). These leukotrienes are potent inflammatory mediators involved in the pathophysiology of asthma and allergic rhinitis.

Onset
Within 1 day for asthma symptoms.
Excretion
Primarily excreted in feces via bile (approximately 86%). Less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in 2-4 hours for adults.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes CYP3A4, CYP2C9, and CYP2C8.

Rifampin
May decrease montelukast plasma concentrations by approximately 40%.
Phenobarbital
May decrease montelukast plasma concentrations by approximately 40%.

In cases of overdose, the most common adverse events were abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Management is symptomatic and supportive. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Montelukast is excreted in breast milk; caution should be exercised when administered to a nursing woman.
