Overview of the medicine
Verigra (Sildenafil Citrate) is a medication used to treat erectile dysfunction (ED) in men. It helps achieve and maintain an erection sufficient for sexual activity by increasing blood flow to the penis.
Uses & Indications
Dosage
Adults
For most patients, the recommended dose is 50 mg taken approximately 1 hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to 100 mg or decreased to 25 mg. The maximum recommended dosing frequency is once per day.
Elderly
No specific dose adjustment is required for elderly patients (≥65 years) if liver and kidney function are normal. However, a starting dose of 25 mg may be considered due to age-related decrease in clearance.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Verigra is taken orally, with or without food. However, a high-fat meal may delay the effect. For optimal effect, it is usually taken on an empty stomach approximately 1 hour before sexual activity.
Mechanism of Action
Sildenafil is a potent and selective inhibitor of cGMP-specific phosphodiesterase type 5 (PDE5) in the corpus cavernosum, where PDE5 is responsible for degradation of cGMP. Sildenafil enhances the effect of nitric oxide by inhibiting PDE5, resulting in increased levels of cGMP, smooth muscle relaxation, and increased blood flow into the corpus cavernosum, thus facilitating an erection.

Onset
Approximately 30-60 minutes.
Excretion
Eliminated predominantly as metabolites in the feces (approximately 80% of the administered oral dose) and to a lesser extent in the urine (approximately 13% of the administered oral dose).
Half life
Approximately 3-5 hours.
Absorption
Rapidly absorbed after oral administration, with mean absolute bioavailability of 41%. Peak plasma concentrations (Cmax) are reached within 30-120 minutes (median 60 minutes) of oral dosing in the fasted state.
Metabolism
Primarily metabolized by hepatic microsomal isoenzymes CYP3A4 (major route) and CYP2C9 (minor route). The major circulating metabolite, N-desmethyl sildenafil, is about 50% as potent as the parent drug.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin) or nitric oxide donors due to potentiation of hypotensive effects.
- Hypersensitivity to sildenafil or any component of the tablet.
- Patients for whom sexual activity is inadvisable due to cardiovascular status.
- Recent history of myocardial infarction, stroke, or life-threatening arrhythmia.
- Known hereditary degenerative retinal disorders, such as retinitis pigmentosa.
Drug Interactions
Nitrates
Potentiation of hypotensive effects, leading to severe hypotension.
Riociguat
Concomitant use is contraindicated due to risk of severe hypotension.
Alpha-blockers (e.g., doxazosin, tamsulosin)
Risk of symptomatic hypotension, especially if taken within 4 hours.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, ritonavir)
Increases sildenafil plasma levels, requiring dose reduction.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In studies with healthy volunteers, doses up to 800 mg resulted in similar adverse events as seen at lower doses, but with increased incidence and severity. In cases of overdose, standard supportive measures should be adopted as sildenafil is highly protein-bound and not expected to be eliminated by renal dialysis.
Pregnancy & Lactation
Verigra is not indicated for use in women. Animal studies did not indicate direct or indirect harmful effects with respect to pregnancy or embryo/fetal development.
