Description
Overview of the medicine
Nimesulide (Versil) is a non-steroidal anti-inflammatory drug (NSAID) used for the short-term treatment of acute pain, symptomatic treatment of osteoarthritis, and primary dysmenorrhea. It exhibits anti-inflammatory, analgesic, and antipyretic properties.
Uses & Indications
Dosage
Adults
100 mg twice daily. Maximum duration of treatment should not exceed 15 days.
Elderly
Similar to adults, but with caution and monitoring due to increased risk of adverse effects.
Renal_impairment
For mild to moderate renal impairment, no dosage adjustment is usually needed. Contraindicated in severe renal impairment.
How to Take
Take orally after meals to minimize gastrointestinal upset. Do not chew or crush the tablet.
Mechanism of Action
Nimesulide selectively inhibits cyclooxygenase-2 (COX-2), leading to reduced synthesis of prostaglandins, which are mediators of inflammation, pain, and fever.

Onset
Analgesic effect usually within 30-60 minutes.
Excretion
Mainly excreted in the urine (around 50-60%) and feces (20-30%).
Half life
Approximately 2-5 hours.
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations are reached within 1-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxynimesulide, which is also pharmacologically active.

Drug Interactions
Lithium
Increased plasma levels of lithium.
Methotrexate

Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, lethargy. Severe cases can lead to gastrointestinal bleeding, acute renal failure, hepatic dysfunction, respiratory depression, and coma. Management is supportive and symptomatic; there is no specific antidote.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to potential risks to the fetus. Avoid during breastfeeding as it may pass into breast milk.
