Description
Overview of the medicine
Solifenacin (Vesinac) is a muscarinic receptor antagonist used to treat symptoms of overactive bladder (OAB), such as urinary urgency, frequency, and urge incontinence.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. May be increased to 10 mg once daily if needed.
Elderly
No dose adjustment generally required, but caution is advised.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min), maximum dose is 5 mg once daily.
How to Take
Take orally with water, with or without food. Swallow the tablet whole; do not chew, crush, or break it.
Mechanism of Action
Solifenacin competitively antagonizes muscarinic M3 receptors, which are primarily responsible for bladder smooth muscle contraction. By inhibiting these receptors, it reduces bladder activity and increases bladder capacity.
Pharmacokinetics
Onset
Clinical effects may be observed within a week, full effect within 4-8 weeks.
Excretion
Primarily excreted in urine (approximately 69%) and feces (approximately 23%).
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract, bioavailability is about 90%. Peak plasma concentrations reached within 3-8 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4).
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Severe liver impairment
- Hypersensitivity to solifenacin or any component of the formulation
Drug Interactions
Cholinergic agonists
May reduce efficacy of solifenacin.
Drugs that prolong QT interval
Use with caution due to potential additive effects.
CYP3A4 inducers (e.g., rifampin)
Decreased solifenacin exposure.
Strong CYP3A4 inhibitors (e.g., ketoconazole)
Increased solifenacin exposure; limit dose to 5 mg/day.
Storage
Store at room temperature (20°C to 25°C), away from moisture and heat. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe anticholinergic effects (e.g., severe dry mouth, blurred vision, tachycardia, urinary retention, dilated pupils). Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown whether solifenacin is excreted in human milk; use with caution in nursing mothers.

Solifenacin has been extensively studied in multiple clinical trials demonstrating its efficacy and safety for OAB symptoms. Key trials include those comparing it to placebo and other antimuscarinics.
Lab Monitoring
- Monitor for signs of urinary retention or decreased renal function periodically.
- Monitor liver function tests in patients with hepatic impairment.
Doctor Notes
- Counsel patients on potential anticholinergic side effects and strategies to manage them.
- Assess for urinary retention before and during therapy.
- Consider drug interactions, especially with strong CYP3A4 inhibitors.
Patient Guidelines
- Take medicine exactly as prescribed.
- Do not stop taking the medicine abruptly.
- Report any new or worsening symptoms to your doctor.
- Avoid activities requiring mental alertness until you know how this medicine affects you.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not take two doses at once.
Driving Precautions
May cause blurred vision, dizziness, or somnolence. Patients should be cautioned about engaging in activities requiring mental alertness, such as driving or operating machinery, until they know how solifenacin affects them.
Lifestyle Advice
- Avoid excessive caffeine and alcohol consumption, as these can irritate the bladder.
- Maintain adequate fluid intake.
