Overview of the medicine
Veslac (Fesoterodine Fumarate) is an extended-release tablet used to treat symptoms of overactive bladder (OAB), such as urge urinary incontinence, urgency, and urinary frequency.
Uses & Indications
Dosage
Adults
Recommended starting dose is 4 mg orally once daily. May be increased to 8 mg once daily based on individual response and tolerability.
Elderly
No dose adjustment is generally required for elderly patients, but caution is advised.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min), the maximum dose is 4 mg once daily. For moderate impairment (CrCl 30-80 mL/min), caution is advised, and max 8 mg.
How to Take
Take orally once daily with or without food. Swallow the tablet whole; do not chew, crush, or divide it.
Mechanism of Action
Fesoterodine is a prodrug that is rapidly and extensively hydrolyzed by plasma esterases to its active metabolite, 5-hydroxymethyltolterodine (5-HMT). 5-HMT is a muscarinic receptor antagonist that blocks muscarinic receptors in the bladder, thereby inhibiting involuntary detrusor contractions.

Primarily excreted via the kidneys (approximately 70% as 5-HMT and its metabolites) and to a lesser extent via feces (approximately 10%).

Absorption
Fesoterodine is rapidly absorbed and extensively hydrolyzed to its active metabolite, 5-HMT. Peak plasma concentrations of 5-HMT are reached within approximately 5 hours.
Metabolism
Rapid and extensive hydrolysis by plasma esterases to 5-HMT. Further metabolism of 5-HMT occurs via CYP2D6 and CYP3A4.

Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered. Physostigmine may be used for severe central anticholinergic effects.

Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether fesoterodine is excreted in human milk; caution should be exercised when administered to a nursing mother.
