Overview of the medicine
Vesodil 25 mg Tablet contains Isosorbide Dinitrate, a vasodilator primarily used to prevent and treat angina pectoris (chest pain) caused by coronary artery disease. It works by relaxing and widening blood vessels, allowing more blood and oxygen to reach the heart.
Uses & Indications
Dosage
Adults
Initial dose: 5-20 mg 2-3 times daily. Maintenance dose: 10-40 mg 2-3 times daily, with an interval of 18 hours free from the drug to prevent tolerance.
Elderly
Lower initial doses may be considered due to increased sensitivity or comorbidities.
Renal_impairment
No specific dose adjustment is usually required, but caution is advised in severe impairment.
How to Take
Take tablets orally with water, with or without food. Do not crush or chew. For acute attacks, sublingual forms are preferred.
Mechanism of Action
Isosorbide Dinitrate is metabolized to nitric oxide (NO), which activates guanylate cyclase, leading to an increase in cyclic GMP (cGMP). Increased cGMP causes dephosphorylation of myosin light chains in smooth muscle, resulting in relaxation of vascular smooth muscle cells. This leads to venodilation (reduced preload) and arterial dilation (reduced afterload), decreasing cardiac workload and improving myocardial oxygen supply.

Primarily excreted renally as metabolites.
Half life
Approximately 1 hour for the parent drug, but active metabolites (isosorbide-2-mononitrate and isosorbide-5-mononitrate) have longer half-lives (1.5-2 hours and 4-6 hours respectively).
Absorption
Well absorbed from the GI tract, but undergoes significant first-pass metabolism, leading to low bioavailability (around 22%).
Metabolism
Extensively metabolized in the liver by denitration to isosorbide mononitrates (active metabolites) and subsequently to inactive glucuronides.
Side Effects
Contraindications

- •Closed-angle glaucoma
- •Concurrent use with phosphodiesterase-5 (PDE5) inhibitors (e.g., sildenafil, tadalafil) due to risk of severe hypotension
- •Increased intracranial pressure
- •Circulatory collapse and severe hypotension
Drug Interactions
Alcohol
Enhances hypotensive effects and may cause syncope.
Antihypertensives
Additive hypotensive effects.
PDE5 Inhibitors (e.g., Sildenafil, Tadalafil)
Profound and prolonged hypotension, potentially fatal. Concomitant use is absolutely contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, tachycardia, flushing, headache, nausea, vomiting, methemoglobinemia. Treatment involves elevating legs, administering IV fluids, and if necessary, vasopressors. For methemoglobinemia, methylene blue may be required.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown if Isosorbide Dinitrate is excreted in breast milk; use with caution in breastfeeding mothers.
