Description
Overview of the medicine
Vestar MR is a cardioprotective agent used for the symptomatic treatment of stable angina pectoris in adults, particularly in patients who are intolerant to or inadequately controlled by first-line antianginal therapies.
Uses & Indications
Dosage
Adults
One 35 mg modified-release tablet twice daily, i.e., once in the morning and once in the evening, during meals.
Elderly
No specific dose adjustment is required in elderly patients. However, caution should be exercised in elderly patients with moderate renal impairment.
Renal_impairment
For patients with moderate renal impairment (creatinine clearance 30-60 ml/min), the recommended dose is one 35 mg modified-release tablet in the morning during breakfast. Severe renal impairment (creatinine clearance < 30 ml/min) is a contraindication.
How to Take
Vestar MR tablets should be taken orally, with meals, and swallowed whole with a glass of water. Do not chew or crush the tablet.
Mechanism of Action
Trimetazidine inhibits the 3-ketoacyl CoA thiolase enzyme in the myocardium, shifting the energy metabolism from fatty acid oxidation to glucose oxidation. This change in metabolism optimizes myocardial energy processes during ischemia and protects cells from the effects of reduced oxygen supply.

Onset
Gradual onset of anti-ischemic effect, typically within a few days of regular administration.
Excretion
Primarily excreted unchanged in the urine (approximately 80% of the dose).

Plasma half-life of approximately 7 hours for the modified-release formulation.

Rapid absorption after oral administration; peak plasma concentration approximately 2 hours for modified-release formulation.

Minor hepatic metabolism; no major active metabolites identified.
