Description
Overview of the medicine
Sildenafil is used to treat erectile dysfunction (ED). It works by increasing blood flow to the penis to help men get and keep an erection when they are sexually aroused.
Uses & Indications
Dosage
Adults
For most patients, the recommended dose is 50 mg taken approximately 30-60 minutes before sexual activity. The dose may be increased to 100 mg or decreased to 25 mg based on efficacy and tolerability. Maximum recommended dosing frequency is once per day.
Elderly
No dosage adjustment required for elderly patients, however, consider starting at 25 mg due to potential for decreased clearance.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min) or hepatic impairment, a starting dose of 25 mg should be considered.
How to Take
Oral administration. Take approximately 30-60 minutes before sexual activity. Can be taken with or without food, but a high-fat meal may delay the onset of action.
Mechanism of Action
Sildenafil selectively inhibits cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5) in the corpus cavernosum of the penis. This results in increased cGMP levels, leading to smooth muscle relaxation and inflow of blood to the penis, causing an erection.

Onset
30-60 minutes.
Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of the administered oral dose) and to a lesser extent in the urine (approximately 13% of the administered oral dose).
Half life
Approximately 3-5 hours.
Absorption
Rapidly absorbed. Maximum plasma concentrations are reached within 30-120 minutes (median 60 minutes) of oral dosing in the fasted state. Mean absolute oral bioavailability is 41%.
Metabolism
Primarily metabolized by the cytochrome P450 (CYP) isoforms 3A4 (major route) and 2C9 (minor route) in the liver.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin, isosorbide dinitrate) due to risk of severe hypotension.
- Concomitant use with riociguat.
- Known hypersensitivity to sildenafil or any component of the tablet.
- Patients with severe cardiovascular disease, recent myocardial infarction or stroke, uncontrolled hypertension or hypotension.
Drug Interactions
Nitrates
Concomitant use is contraindicated due to synergistic hypotensive effects, leading to severe hypotension.
Alpha-blockers
May lead to symptomatic hypotension in some patients. Initiate Sildenafil with caution at 25 mg.
CYP3A4 Inhibitors
Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, ritonavir) may increase sildenafil plasma levels, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from moisture and direct light. Keep out of reach of children.
Overdose
In studies with healthy volunteers of single doses up to 800 mg, adverse events were similar to those seen at lower doses but incidence rates were increased. In cases of overdose, standard supportive measures should be adopted as required.
Pregnancy & Lactation
Not indicated for use in women. There are no adequate and well-controlled studies of sildenafil in pregnant or lactating women.
