Viagrazen 50 mg Tablet contains Sildenafil Citrate, a phosphodiesterase-5 (PDE5) inhibitor. It is primarily used to treat erectile dysfunction (impotence) in men by increasing blood flow to the penis during sexual stimulation. It helps achieve and maintain an erection.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately one hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to 100 mg or decreased to 25 mg. The maximum recommended dose is 100 mg once daily.
Elderly
No dosage adjustment is generally required for elderly patients, but a starting dose of 25 mg should be considered due to potential age-related decline in renal and hepatic function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take the tablet orally with water, approximately one hour before planned sexual activity. It can be taken with or without food, but a high-fat meal may delay the onset of action. Do not take more than one dose per day.
Mechanism of Action
Sildenafil inhibits phosphodiesterase type 5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. By inhibiting PDE5, Sildenafil increases cGMP levels, leading to smooth muscle relaxation and increased blood flow into the penis during sexual stimulation, thus facilitating an erection.

Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of the administered oral dose) and, to a lesser extent, in the urine (approximately 13% of the administered oral dose).

Absorption
Rapidly absorbed. Mean absolute bioavailability is about 41%. Peak plasma concentrations are reached within 30-120 minutes (median 60 minutes) of oral dosing in the fasted state.
Metabolism
Primarily metabolized by CYP3A4 (major route) and CYP2C9 (minor route) in the liver. The major circulating metabolite, N-desmethyl sildenafil, has about 50% of the activity of sildenafil.
Side Effects
Contraindications
- Concomitant use of nitrates in any form (e.g., nitroglycerin, isosorbide dinitrate) due to the risk of severe hypotension.
- Concomitant use of guanylate cyclase stimulators (e.g., riociguat).
- Patients with known hypersensitivity to Sildenafil or any component of the tablet.
- Patients for whom sexual activity is inadvisable due to underlying cardiovascular conditions.
- Recent stroke, myocardial infarction, or life-threatening arrhythmia.
- Severe hepatic impairment.
- Known hereditary degenerative retinal disorders (e.g., retinitis pigmentosa).
- Loss of vision in one eye due to non-arteritic anterior ischemic optic neuropathy (NAION).
Drug Interactions
Alcohol
Excessive alcohol consumption may increase the risk of orthostatic hypotension.
Nitrates
Potentiation of hypotensive effects; concomitant use is contraindicated.
Riociguat
Concomitant use is contraindicated due to increased risk of hypotension.
Alpha-blockers
Potential for symptomatic hypotension. Initiate Sildenafil at 25 mg in patients on alpha-blocker therapy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir, Erythromycin)
Can significantly increase Sildenafil plasma levels. A starting dose of 25 mg should be considered in these patients.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In studies with healthy volunteers, single doses up to 800 mg have been given. Adverse reactions were similar to those seen at lower doses but generally increased in incidence and severity. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly bound to plasma proteins and not eliminated in the urine.
Pregnancy & Lactation
Viagrazen is not indicated for use in women. There are no adequate and well-controlled studies of Sildenafil in pregnant or lactating women.
