Overview of the medicine
Viamax 100 mg tablet contains Sildenafil Citrate, primarily used to treat erectile dysfunction (impotence) in men. It works by increasing blood flow to the penis during sexual stimulation. It may also be used to treat pulmonary arterial hypertension (PAH) in some cases.
Uses & Indications
Dosage
Adults
For Erectile Dysfunction: The recommended dose is 50 mg taken approximately 1 hour before sexual activity. It may be increased to 100 mg or decreased to 25 mg based on efficacy and tolerability. Maximum recommended dosing frequency is once per day.
Elderly
A starting dose of 25 mg should be considered in elderly patients (>65 years) due to increased plasma levels.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take orally with a glass of water, with or without food, approximately 30-60 minutes before anticipated sexual activity. A high-fat meal may delay the onset of action.
Mechanism of Action
Sildenafil is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5) in the corpus cavernosum. When sexual stimulation causes local release of nitric oxide (NO), PDE5 inhibition by sildenafil increases cGMP levels, resulting in smooth muscle relaxation and inflow of blood into the corpus cavernosum, thus aiding erection.
Pharmacokinetics
Onset
Onset of action typically occurs within 30-60 minutes after oral administration.
Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of administered oral dose) and to a lesser extent in the urine (approximately 13% of administered oral dose).
Half life
Mean terminal half-life is approximately 3-5 hours.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is about 41%. Peak plasma concentrations (Tmax) are reached within 30 to 120 minutes (median 60 minutes).
Metabolism
Primarily metabolized by hepatic CYP3A4 (major route) and CYP2C9 (minor route) enzymes.
Side Effects
Contraindications
- •Concomitant use of nitrates (e.g., nitroglycerin, isosorbide dinitrate) in any form, as it can cause a significant drop in blood pressure.
- •Known hypersensitivity to Sildenafil or any component of the tablet.
- •Patients for whom sexual activity is inadvisable due to cardiovascular status.
- •Recent stroke or myocardial infarction.
- •Severe hepatic impairment, hypotension (<90/50 mmHg), recent stroke or myocardial infarction, and known hereditary degenerative retinal disorders.
Drug Interactions
Nitrates
Potentiation of hypotensive effects; contraindicated.
Riociguat
Concomitant use is contraindicated due to risk of symptomatic hypotension.
Alpha-blockers
Increased risk of symptomatic hypotension; cautious use with lower starting dose.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Increased sildenafil plasma levels; reduced sildenafil dose may be necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In studies with healthy volunteers of single doses up to 800 mg, adverse events were similar to those seen at lower doses but incidence rates were increased. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly protein bound and not eliminated in the urine.
Pregnancy & Lactation
Viamax is not indicated for use in women. There are no adequate and well-controlled studies of sildenafil in pregnant or lactating women.
