Overview of the medicine
Sildenafil citrate 50 mg is an oral therapy for erectile dysfunction, which helps men achieve and maintain an erection suitable for sexual activity.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately 1 hour before sexual activity. The dose may be increased to 100 mg or decreased to 25 mg based on efficacy and tolerability. Maximum recommended dosing frequency is once per day.
Elderly
No dosage adjustment is required in elderly patients. However, a starting dose of 25 mg may be considered due to potential age-related decrease in clearance.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered. No dosage adjustment is required for mild to moderate renal impairment.
How to Take
For oral administration. Take the tablet whole with water, with or without food, approximately 1 hour before planned sexual activity.
Mechanism of Action
Sildenafil selectively inhibits cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5), which is responsible for the degradation of cGMP in the corpus cavernosum. Inhibition of PDE5 increases cGMP levels, leading to smooth muscle relaxation and increased blood flow into the penis, resulting in an erection with sexual stimulation.

Onset
30-60 minutes.
Excretion
Excreted predominantly as metabolites in feces (approximately 80% of administered oral dose) and to a lesser extent in urine (approximately 13% of administered oral dose).
Half life
Approximately 3-5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 30-120 minutes. Absolute bioavailability is approximately 41%.
Metabolism
Primarily metabolized by hepatic microsomal isoenzymes CYP3A4 (major route) and CYP2C9 (minor route). The major circulating metabolite, N-desmethyl sildenafil, has about 50% of the activity of sildenafil.
Side Effects
Contraindications
- •Concomitant use with nitrates or nitric oxide donors (e.g., nitroglycerin) due to potential for severe hypotension.
- •Hypersensitivity to sildenafil or any component of the tablet.
- •Patients for whom sexual activity is inadvisable due to cardiovascular risk factors.
- •Recent stroke or myocardial infarction, severe hepatic impairment, hypotension (<90/50 mmHg), hereditary degenerative retinal disorders.
Drug Interactions
Nitrates
Potentiation of hypotensive effects, leading to severe and potentially fatal hypotension. Concomitant use is absolutely contraindicated.
Riociguat
Concomitant use is contraindicated due to increased risk of hypotension.
Alpha-blockers
May lead to symptomatic hypotension in some patients, especially within 4 hours of sildenafil administration.
CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole, Erythromycin)
Increase sildenafil plasma concentrations, requiring a reduced starting dose of sildenafil.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In studies of healthy volunteers given single doses up to 800 mg, adverse events were similar to those seen at lower doses but incidence rates were increased. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly bound to plasma proteins and not eliminated in the urine.
Pregnancy & Lactation
Viamax is not indicated for use in women, including during pregnancy or lactation.
