Overview of the medicine
Viera 50 mg Tablet contains Sildenafil Citrate, a phosphodiesterase-5 (PDE5) inhibitor, used to treat erectile dysfunction by increasing blood flow to the penis during sexual stimulation.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately 1 hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to 100 mg or decreased to 25 mg. Maximum recommended dosing frequency is once per day.
Elderly
No dosage adjustment is generally required for elderly patients (≥65 years).
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take the tablet orally with water, approximately 1 hour before sexual activity. Can be taken with or without food, but food (especially high-fat meals) may delay the onset of action.
Mechanism of Action
Sildenafil selectively inhibits phosphodiesterase type 5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. By inhibiting PDE5, Sildenafil increases cGMP levels, leading to smooth muscle relaxation and enhanced blood flow into the penis, resulting in an erection upon sexual stimulation.

Typically 30-60 minutes after oral administration.
Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of administered oral dose) and to a lesser extent in the urine (approximately 13% of administered oral dose).
Half life
Approximately 3-5 hours.
Absorption
Rapidly absorbed. Mean absolute bioavailability of about 41%. Peak plasma concentrations are reached within 30-120 minutes (median 60 minutes) of oral dosing in the fasted state.
Metabolism
Primarily metabolized by CYP3A4 (major route) and CYP2C9 (minor route) in the liver. The major circulating metabolite, N-desmethyl sildenafil, has similar PDE5 selectivity to sildenafil and accounts for approximately 20% of the pharmacological activity.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin), due to the risk of severe hypotension.
- Concomitant use with guanylate cyclase stimulators (e.g., riociguat).
- Patients with known hypersensitivity to sildenafil or any component of the tablet.
- Patients who have suffered a recent myocardial infarction, stroke, or life-threatening arrhythmia.
- Patients with severe hepatic impairment.
- Patients with hereditary degenerative retinal disorders, such as retinitis pigmentosa.
- Patients with hypotension (<90/50 mmHg).

Alcohol
Excessive alcohol intake may increase the risk of orthostatic hypotension.
Nitrates
Contraindicated due to potentiation of hypotensive effects.
Riociguat
Contraindicated due to risk of severe hypotension.
Alpha-blockers (e.g., prazosin)
Increased risk of symptomatic hypotension; initiate sildenafil at 25 mg.
CYP3A4 Inhibitors (e.g., ritonavir, ketoconazole, erythromycin)
Increased plasma levels of sildenafil; reduce sildenafil dose.

Overdose
In studies with healthy volunteers of single doses up to 800 mg, adverse events were similar to those seen at lower doses but incidence rates were increased. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly protein bound and not eliminated in the urine.
