Description
Overview of the medicine
Vinca-10-mg-tablet is an oral antineoplastic agent containing Vinorelbine, a vinca alkaloid. It is used in the treatment of various cancers by interfering with cell division.
Uses & Indications
Dosage
Adults
Typically 60 mg/m² orally once weekly for 3 weeks followed by one week rest, adjusted based on tolerability and blood counts. Max 80 mg/m² for subsequent cycles.
Elderly
No specific dose adjustment based solely on age, but close monitoring of toxicity.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Administer orally, with or without food. Swallow whole, do not chew, crush, or open the tablet.
Mechanism of Action
Vinorelbine binds to tubulin, inhibiting its polymerization into microtubules. This disrupts the formation of the mitotic spindle, leading to metaphase arrest and subsequent apoptosis of cancer cells.
Pharmacokinetics
Onset
Not immediate; therapeutic effects are observed over cycles of treatment.
Excretion
Mainly excreted via the bile and feces, with a small portion via urine.
Half life
Terminal half-life is approximately 27-40 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 40%.
Metabolism
Primarily metabolized in the liver by CYP3A4 to active and inactive metabolites.
Side Effects
Contraindications
- Severe neutropenia (<1500 cells/mm³)
- Severe thrombocytopenia
- Pregnancy and breastfeeding
- Severe hepatic impairment
Drug Interactions
Live vaccines
Avoid due to immunosuppression.
CYP3A4 inducers (e.g., phenytoin, rifampicin)
May decrease Vinorelbine levels and efficacy.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase Vinorelbine levels and toxicity.
Storage
Store below 25°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe myelosuppression, peripheral neurotoxicity, and gastrointestinal toxicity. Management is supportive care, including blood product transfusions and G-CSF for neutropenia.
