Description
Overview of the medicine
Vinca 20 mg Tablet contains Vinorelbine, a chemotherapy drug used to treat various types of cancer, including non-small cell lung cancer and breast cancer. It works by inhibiting cell division.
Uses & Indications
Dosage
Adults
Typically 60 mg/m² body surface area once weekly for 3 weeks, followed by a one-week rest period. Dosage may be adjusted based on hematological counts and patient tolerability. Max single dose 80 mg/m².
Elderly
No specific dose adjustment is generally required based on age alone, but caution should be exercised due to potential age-related decrease in hepatic or renal function.
Renal_impairment
No specific dose adjustment for mild to moderate renal impairment. Use with caution in severe renal impairment, monitor closely.
How to Take
Take the tablet orally with water. Do not chew, crush, or dissolve the tablet. It can be taken with or without food. Wash hands thoroughly after handling.
Mechanism of Action
Vinorelbine is a vinca alkaloid that selectively interferes with microtubule polymerization, primarily affecting mitotic microtubules. This leads to mitotic arrest at metaphase and induces apoptosis, thereby inhibiting cancer cell growth.
Pharmacokinetics
Onset
Onset of action is variable, depending on the disease and regimen. Clinical effects are observed over days to weeks.
Excretion
Excretion occurs primarily via bile and feces (50-70%), with a small amount (11-12%) excreted renally.
Half life
Terminal half-life is approximately 27.7 to 43.6 hours.
Absorption
Oral bioavailability is approximately 27-40%. Peak plasma concentrations are reached within 1.5-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to several metabolites including 4-O-deacetylvinorelbine (an active metabolite).
Side Effects
Contraindications
- Severe neutropenia (<1500 cells/mm³)
- Severe hepatic impairment (Child-Pugh C)
- Pregnancy and lactation
- Hypersensitivity to vinorelbine or other vinca alkaloids
- Serious infection or recent vaccination with live vaccines
Drug Interactions
Live attenuated vaccines
Increased risk of severe or fatal infections in immunocompromised patients.
Other myelosuppressive agents
Increased risk of severe myelosuppression.
CYP3A4 inducers (e.g., Phenytoin, Rifampicin)
May decrease vinorelbine plasma concentrations and efficacy.
CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin)
May increase vinorelbine plasma concentrations and toxicity.
Storage
Store below 25°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe myelosuppression with fever and infection, and severe neurological symptoms. Management involves supportive care, including growth factors for neutropenia, and monitoring vital signs.
Pregnancy & Lactation
Vinorelbine is contraindicated during pregnancy due to potential fetal harm. Women of childbearing potential should use effective contraception. It is also contraindicated during lactation as it may pass into breast milk and harm the infant.
