Overview of the medicine
Vini 20 mg Tablet contains Vinpocetine, a synthetic derivative of the Vinca alkaloid vincamine. It is primarily used to improve cerebral blood flow, oxygen utilization, and neuronal metabolism, thereby enhancing cognitive functions.
Uses & Indications
Dosage
Adults
Initial treatment: 10-20 mg three times daily, usually 20 mg three times daily. Maintenance dose: 10 mg three times daily. Should be taken with or after meals.
Elderly
No specific dose adjustment is usually required for elderly patients, but caution is advised in those with severe underlying conditions.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment as data is limited.
How to Take
Oral administration. Tablets should be swallowed whole with a sufficient amount of liquid, preferably with or after meals to enhance absorption and minimize gastrointestinal discomfort.
Mechanism of Action
Vinpocetine inhibits phosphodiesterase type 1 (PDE1), leading to increased cyclic guanosine monophosphate (cGMP) levels within cerebral smooth muscle cells. This results in vasodilation, improved cerebral microcirculation, and enhanced oxygen and glucose uptake by brain tissues. It also has neuroprotective, anti-inflammatory, and antioxidant properties.

Excretion
Mainly excreted renally (60%) and fecally (40%), primarily as metabolites. Less than 5% is excreted unchanged.

Rapidly absorbed after oral administration; however, absolute bioavailability is low (approx. 7%). Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via hydrolysis to apovincaminic acid (AVA) and subsequent demethylation. Several active and inactive metabolites are formed.
