Description
Overview of the medicine
Viset-10 mg syrup contains Cetirizine, an antihistamine used to relieve symptoms of seasonal and perennial allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps reduce sneezing, runny nose, watery eyes, and itching.
Uses & Indications
Dosage
Adults
Adults and adolescents 12 years and over: 10 ml (10 mg) once daily or 5 ml (5 mg) twice daily.
Elderly
A dose of 5 ml (5 mg) once daily is recommended in elderly patients, especially those with impaired renal function.
Renal_impairment
Dosage should be adjusted according to creatinine clearance: CrCl 31-60 ml/min: 5 ml (5 mg) once daily. CrCl 11-30 ml/min: 5 ml (5 mg) every other day. CrCl <10 ml/min: Contraindicated.
How to Take
Viset-10 mg syrup is for oral administration. It can be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Cetirizine is a potent and highly selective H1-receptor antagonist. It works by blocking the action of histamine, a natural substance in the body that causes allergic symptoms. It also inhibits the migration of inflammatory cells and the release of mediators associated with the late phase of allergic reactions.
Pharmacokinetics
Onset
Onset of action is usually within 20-60 minutes.
Excretion
Approximately 70% of the dose is excreted in the urine, mainly as unchanged drug, and about 10% is excreted in the feces.
Half life
The plasma elimination half-life is approximately 8-11 hours in adults.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are achieved within approximately 1 hour.
Metabolism
Undergoes limited first-pass metabolism; approximately 10-15% of the dose is metabolized. The main metabolic pathway is O-dealkylation to a pharmacologically inactive metabolite.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives
- Patients with severe renal impairment (creatinine clearance less than 10 ml/min)
- Lactating mothers (not absolute, but generally advised to avoid)
Drug Interactions
Ritonavir
Ritonavir may increase cetirizine exposure, potentially increasing side effects.
Theophylline
Concomitant administration with theophylline (400 mg once daily) may result in a small decrease in cetirizine clearance, but minimal clinical significance.
CNS Depressants (e.g., alcohol, sedatives)
Concomitant use may increase drowsiness and impair central nervous system function. Avoid alcohol.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, sedation, tachycardia, fatigue, headache, and tremors. In case of overdose, symptomatic and supportive treatment is recommended. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cetirizine is excreted in breast milk; therefore, use with caution in nursing mothers and consider the risk-benefit.
