Description
Overview of the medicine
Viset 50 mg Tablet contains Sertraline, an antidepressant used to treat various mental health conditions. It belongs to a class of drugs called Selective Serotonin Reuptake Inhibitors (SSRIs) that help restore the balance of serotonin in the brain.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily, taken in the morning or evening. Dosage may be increased gradually, typically in 25 mg or 50 mg increments, at intervals of at least one week, up to a maximum of 200 mg per day.
Elderly
Similar to adult dosing, but lower initial doses (e.g., 25 mg) and slower titration may be considered due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Caution and monitoring are advised for severe renal impairment.
How to Take
Take Viset 50 mg Tablet orally, once daily, with or without food. It is recommended to take it at the same time each day to maintain consistent blood levels. Do not crush, chew, or break the tablet.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by presynaptic neurons in the central nervous system, thereby enhancing serotonergic neurotransmission in the brain. This leads to an increase in the concentration of serotonin in the synaptic cleft, which is believed to be responsible for its antidepressant, anti-obsessive, and anti-panic effects.
Pharmacokinetics
Onset
Antidepressant effects usually observed after 2-4 weeks of treatment; anxiolytic effects may appear sooner.
Excretion
Excreted almost equally in feces and urine as metabolites; less than 0.2% excreted unchanged in urine.
Half life
Approximately 26 hours (range 22-36 hours) for parent compound; N-desmethylsertraline (active metabolite) has a longer half-life of 62-104 hours.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached within 4.5-8.4 hours. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2B6, CYP2C19, CYP2D6, CYP3A4, and CYP2C9, to N-desmethylsertraline and other inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to sertraline or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment (risk of serotonin syndrome).
- Concomitant use with pimozide.
Drug Interactions
MAOIs
Concurrent use is contraindicated due to risk of serotonin syndrome.
Pimozide
Concurrent use is contraindicated.
Warfarin
May enhance anticoagulant effect; monitor INR.
NSAIDs/Aspirin
Increased risk of bleeding, especially gastrointestinal bleeding.
Other Serotonergic Drugs
Increased risk of serotonin syndrome (e.g., other SSRIs, SNRIs, tricyclic antidepressants, fentanyl, lithium, tramadol, St. John's Wort).
Triptans (e.g., Sumatriptan)
Increased risk of serotonin syndrome.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, seizures or serotonin syndrome. Management is primarily supportive; maintain airway, monitor cardiac and vital signs, and provide symptomatic treatment. Gastric lavage and activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs late in the third trimester have developed complications requiring prolonged hospitalization, respiratory support, and tube feeding. Sertraline is excreted in breast milk; caution should be exercised when administered to a nursing woman.
