Overview of the medicine
Visnor (Bisoprolol Fumarate) is a beta-blocker primarily used to treat high blood pressure, angina (chest pain), and chronic heart failure. It works by slowing down the heart rate and relaxing blood vessels, thereby improving blood flow.
Uses & Indications
Dosage
Adults
Typically 5-10 mg once daily, maximum 20 mg/day for hypertension/angina. For heart failure, start with 1.25 mg once daily, titrate up to 10 mg once daily.
Elderly
No specific dosage adjustment needed unless severe renal or hepatic impairment.
Renal_impairment
CrCl < 20 mL/min: Initial 2.5 mg/day, maximum 10 mg/day.
How to Take
Oral administration, usually once daily in the morning with or without food. Swallow whole, do not chew or crush.
Mechanism of Action
Bisoprolol is a highly cardioselective beta-1 adrenergic blocking agent. It selectively blocks beta-1 adrenergic receptors in the heart, leading to decreased heart rate, myocardial contractility, and cardiac output, thus lowering blood pressure.

Onset
Antihypertensive effect seen within 2-3 hours; full effect within 2-4 weeks.
Excretion
Approximately 50% excreted unchanged in urine, and metabolites are also excreted renally and fecally.
Half life
Approximately 9-12 hours.
Absorption
Well absorbed orally, bioavailability approx. 80-90%. Peak plasma concentrations within 2-4 hours.
Metabolism
Metabolized in the liver (approx. 50%) by CYP3A4.
Side Effects
Contraindications
- •Acute heart failure or decompensated heart failure requiring intravenous inotropic therapy
- •Cardiogenic shock
- •Second or third degree AV block without a pacemaker
- •Sick sinus syndrome without a pacemaker
- •Sinoatrial block
- •Symptomatic bradycardia (< 50 bpm)
- •Severe asthma or severe chronic obstructive pulmonary disease
- •Severe peripheral arterial occlusive disease
- •Metabolic acidosis
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of bisoprolol.
Insulin and oral antidiabetics
May mask symptoms of hypoglycemia (e.g., tachycardia).
Antiarrhythmic drugs (e.g., Class I antiarrhythmics)
May potentiate effects on AV conduction.
Calcium channel blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and hypotension.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency. Management involves supportive therapy, IV atropine for bradycardia, IV glucagon, IV beta-stimulants for cardiac depression, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Bisoprolol is excreted in breast milk; caution should be exercised when administered to a nursing woman.
