Description
Overview of the medicine
Pyridoxine (Vitamin B6) is a water-soluble vitamin essential for various metabolic processes. It plays a crucial role in amino acid metabolism, neurotransmitter synthesis, and red blood cell formation.
Uses & Indications
Dosage
Adults
Typically 5-10 mg daily for deficiency, up to 100-200 mg daily for drug-induced neuropathy or certain conditions under medical supervision.
Elderly
Similar to adult dosage, dose adjustments may be needed based on renal function.
Renal_impairment
No specific adjustments usually required for mild to moderate impairment, but caution with high doses in severe impairment.
How to Take
Orally, with or without food.
Mechanism of Action
Pyridoxine acts as a coenzyme in numerous enzymatic reactions, particularly in amino acid and glycogen metabolism. It is converted to its active forms, pyridoxal phosphate (PLP) and pyridoxamine phosphate (PMP), which are involved in over 100 enzyme reactions, mainly deamination, decarboxylation, transamination, and racemization.
Pharmacokinetics
Onset
Effects are gradual, related to correction of deficiency.
Excretion
Excreted in the urine, primarily as 4-pyridoxic acid, an inactive metabolite.
Half life
Approximately 15-20 days for total body stores; plasma half-life of PLP is about 0.5-3 hours.
Absorption
Readily absorbed from the gastrointestinal tract, primarily in the jejunum.
Metabolism
Primarily metabolized in the liver to pyridoxal phosphate (PLP) and other active metabolites.
Side Effects
Contraindications
- Hypersensitivity to Pyridoxine or any component of the formulation.
- Rarely, severe peripheral neuropathy with very high doses over prolonged periods.
Drug Interactions
Levodopa
Pyridoxine can decrease the effectiveness of levodopa (without carbidopa/benserazide) by increasing its peripheral metabolism.
Isoniazid, Hydralazine, Penicillamine, Oral contraceptives
These drugs can increase pyridoxine requirements or interfere with its metabolism, leading to deficiency.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe peripheral neuropathy, ataxia, and sensory deficits. Management is supportive, including discontinuation of the drug and symptomatic treatment.
Pregnancy & Lactation
Generally considered safe at recommended daily allowances. High doses should be avoided during pregnancy and lactation unless clearly indicated by a physician.

Lab Monitoring
- Plasma pyridoxal phosphate (PLP) levels (for deficiency diagnosis/monitoring)
- Liver function tests (with very high doses)
- Renal function tests
Doctor Notes
- Counsel patients on potential interaction with levodopa.
- Monitor for signs of peripheral neuropathy with high-dose, long-term therapy.
- Emphasize not exceeding recommended doses for OTC use.
Patient Guidelines
- Take as directed by your doctor or as per package instructions.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Generally considered safe, but if you experience dizziness or drowsiness (uncommon), avoid driving or operating heavy machinery.
Lifestyle Advice
- Maintain a balanced diet rich in B vitamins (e.g., meat, fish, poultry, potatoes, fruits).
- Avoid excessive alcohol consumption, which can deplete B6 levels.
