Description
Overview of the medicine
Vitrocin is a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract, skin, and sexually transmitted infections.
Uses & Indications
Dosage
Adults
Oral: 250-500 mg every 6-12 hours, depending on the severity of infection. Duration usually 7-14 days.
Elderly
No specific dose adjustment for elderly patients with normal renal and hepatic function.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), consider 50-75% of normal dose.
How to Take
Oral tablets/suspension can be taken with or without food. However, for better absorption, some forms are best taken on an empty stomach 1 hour before or 2 hours after meals. IV administration should be slow infusion.
Mechanism of Action
It inhibits bacterial protein synthesis by binding reversibly to the 50S ribosomal subunit of susceptible microorganisms.

Onset of action within 1-4 hours.
Excretion
Mainly excreted in bile; a small amount is excreted in urine.
Half life
Approximately 1.5 to 2 hours.
Absorption
Variable absorption, improved by enteric coating or ester forms. Bioavailability is around 30-65%.

Primarily hepatic via CYP3A4 enzyme system.
Side Effects
Contraindications
- Hypersensitivity to Vitrocin or any other macrolide antibiotics.
- Co-administration with cisapride, pimozide, ergotamine, dihydroergotamine due to risk of life-threatening arrhythmias.
- History of cholestatic jaundice/hepatic dysfunction associated with Vitrocin use.
Drug Interactions
Warfarin
Increased anticoagulant effect, monitor INR.
Theophylline, Carbamazepine, Digoxin
Increased levels of these drugs, monitor concentrations.
Statins (e.g., Simvastatin, Lovastatin)
Increased risk of myopathy/rhabdomyolysis.
CYP3A4 inhibitors (e.g., Azole antifungals, Protease inhibitors)
Increased Vitrocin levels.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose include severe nausea, vomiting, abdominal cramps, and diarrhea. Management includes gastric lavage, activated charcoal, and symptomatic supportive care. Hemodialysis is not effective.

Pregnancy Category B. Generally considered safe, but use only if clearly needed. Excreted in breast milk in small amounts; generally compatible with breastfeeding, but monitor infant for GI side effects.
