Description
Overview of the medicine
Vivanta 97 mg Tablet is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily and thereby lowering blood pressure.
Uses & Indications
Dosage
Adults
Initial dose: 80 mg once daily. May be increased to 160 mg or 320 mg once daily based on blood pressure response. For heart failure, initial dose is 40 mg twice daily.
Elderly
No initial dosage adjustment required for elderly patients. Close monitoring is recommended.
Renal_impairment
No initial dosage adjustment required for mild to moderate renal impairment. For severe impairment (CrCl < 30 mL/min), lower initial doses may be considered, and careful monitoring is essential.
How to Take
Take Vivanta 97 mg Tablet orally with or without food, usually once daily. It is important to take it regularly at the same time each day for best results. Do not crush, chew, or break the tablet; swallow it whole.
Mechanism of Action
Vivanta selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues (e.g., vascular smooth muscle, adrenal gland, kidney). This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased aldosterone secretion, and reduced blood pressure.
Pharmacokinetics
Onset
Within 2 hours, full effect achieved within 2-4 weeks.
Excretion
Excreted primarily in feces (83%) and urine (13%) as unchanged drug.
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-4 hours. Absolute bioavailability is approximately 25-30%.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Vivanta or any component of the formulation.
- Pregnancy (second and third trimesters).
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported. Monitor lithium levels.
Aliskiren
Contraindicated in diabetic patients or those with renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment, especially in elderly or volume-depleted patients.
Potassium-sparing diuretics (e.g., spironolactone)
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked hypotension and possibly tachycardia or bradycardia. Treatment is supportive and symptomatic. If ingestion is recent, induce emesis or perform gastric lavage. Administer activated charcoal. Closely monitor blood pressure and vital signs.
Pregnancy & Lactation
Vivanta is contraindicated during the second and third trimesters of pregnancy due to risks of fetal injury. It is not recommended during lactation, as it is unknown if it is excreted in human milk. Consult your doctor for safe alternatives if you are pregnant, planning to become pregnant, or breastfeeding.
