Description
Overview of the medicine
Volibo is an oral antidiabetic medicine containing Voglibose. It is used to manage post-prandial hyperglycemia in patients with Type 2 Diabetes Mellitus by delaying carbohydrate digestion and absorption.
Uses & Indications
Dosage
Adults
Initial dose: 0.2 mg three times daily, taken immediately before each main meal. The dose may be increased to 0.3 mg three times daily if necessary, after monitoring blood glucose and tolerance. Maximum recommended dose is 0.9 mg/day.
Elderly
No specific dose adjustment is generally required for elderly patients, but use with caution and monitor for gastrointestinal side effects.
Renal_impairment
Use with caution in mild to moderate renal impairment. Not recommended in severe renal impairment (creatinine clearance < 30 mL/min) due to lack of data.
How to Take
Volibo tablets should be taken orally, immediately before each main meal (breakfast, lunch, dinner). The tablet should be swallowed whole with water, not chewed or crushed.
Mechanism of Action
Voglibose competitively inhibits alpha-glucosidase enzymes (like sucrase, maltase, glucoamylase) present in the brush border of the small intestine. This action delays the digestion of complex carbohydrates into absorbable monosaccharides, leading to a slower and lower rise in post-meal blood glucose.
Pharmacokinetics
Onset
Rapid, reduction in post-prandial glucose seen within 1-2 hours of administration.
Excretion
Primarily excreted unchanged via feces. A very small fraction is excreted renally.
Half life
Very short, typically less than 2 hours in plasma due to minimal systemic absorption.
Absorption
Poorly absorbed from the gastrointestinal tract, with systemic bioavailability of less than 1%.
Metabolism
Undergoes minimal metabolism within the gastrointestinal tract; not significantly metabolized by hepatic enzymes.
Side Effects
Contraindications
- Diabetic ketoacidosis.
- Inflammatory bowel disease (Crohn's disease, ulcerative colitis).
- Chronic intestinal diseases associated with marked disorders of digestion or absorption.
- Large hernia.
- Severe infections.
- Severe renal or hepatic impairment.
- Hypersensitivity to Voglibose or any component of the formulation.
- Pregnancy and lactation.
Drug Interactions
Other oral antidiabetics
Concomitant use may enhance hypoglycemic effect.
Sulfonylureas, Insulin, Metformin
Increased risk of hypoglycemia, requiring dose adjustment of these agents.
Adsorbents (e.g., activated charcoal), Enzyme preparations (e.g., amylase, pancreatin)
May reduce the effect of Voglibose.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to exaggerated gastrointestinal side effects such as increased flatulence, abdominal pain, and diarrhea. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Voglibose is not recommended during pregnancy and lactation due to insufficient data on safety. Consult your doctor for alternative treatments if you are pregnant, planning to become pregnant, or breastfeeding.

Clinical Trials
Voglibose has been extensively studied in clinical trials for its efficacy and safety in managing post-prandial hyperglycemia in Type 2 Diabetes. Ongoing research explores its potential in other metabolic conditions and in combination therapies.
Lab Monitoring
- Periodic monitoring of blood glucose levels (fasting and post-prandial).
- HbA1c levels to assess long-term glycemic control.
- Liver function tests (ALT, AST) periodically, especially in patients with pre-existing hepatic conditions.
Doctor Notes
- Emphasize strict adherence to meal-time administration to maximize efficacy and minimize side effects.
- Counsel patients on potential gastrointestinal side effects and advise on dietary adjustments to manage them.
- Regular monitoring of blood glucose and liver function tests is crucial for long-term management.
Patient Guidelines
- Take Volibo exactly as prescribed by your doctor.
- Always take the tablet immediately before each main meal.
- Do not skip meals while taking this medicine.
- Maintain a balanced diet and regular exercise routine as advised by your healthcare provider.
- Be aware of symptoms of hypoglycemia, especially if taking with other diabetes medications.
Missed Dose Advice
If you miss a dose, take it as soon as you remember before your next meal. If you remember after the meal, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Volibo itself is unlikely to cause hypoglycemia when used alone and thus generally does not impair the ability to drive or operate machinery. However, if used in combination with other antidiabetic agents that can cause hypoglycemia, patients should be cautious about driving or operating machinery.
Lifestyle Advice
- Adhere strictly to dietary recommendations, focusing on complex carbohydrates and limiting simple sugars.
- Regular physical activity is crucial for blood glucose control.
- Monitor blood glucose regularly as advised by your doctor.
