Description
Overview of the medicine
Volifen 1-V Spray contains Diclofenac Diethylamine, a non-steroidal anti-inflammatory drug (NSAID), used for localized pain and inflammation, such as those associated with muscle sprains, strains, and minor joint pain. It works by reducing swelling and pain directly at the site of application.
Uses & Indications
Dosage
Adults
Apply 3-4 times daily to the affected area. Spray approximately 2-4 puffs depending on the size of the affected area.
Elderly
No specific dosage adjustment needed for elderly patients; however, caution is advised due to potentially thinner skin.
Renal_impairment
No specific dosage adjustment needed due to minimal systemic absorption.
How to Take
Spray a small amount of the solution onto the affected painful area 3 to 4 times daily and gently rub in, if necessary. Wash hands thoroughly after application. Do not apply on open wounds or irritated skin.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX-1 and COX-2) enzymes. Prostaglandins are mediators of pain and inflammation. When applied topically, it penetrates the skin to deliver the active ingredient directly to the affected tissues, providing localized anti-inflammatory and analgesic effects.
Pharmacokinetics
Onset
Within 30-60 minutes for local pain relief.
Excretion
Systemically absorbed drug is primarily excreted renally (urine) as metabolites, with some biliary excretion.
Half life
Systemic half-life of diclofenac is approximately 1-2 hours, but local tissue concentration may persist longer.
Absorption
Transdermal absorption, variable depending on skin condition, with minimal systemic uptake.
Metabolism
Primarily hepatic (CYP2C9, CYP2C8, CYP2C19) if systemically absorbed; local metabolism also possible.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, other NSAIDs, or any excipients
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Application on open wounds, infected skin, or eczema
- Third trimester of pregnancy
Drug Interactions
Oral anticoagulants
Although systemic absorption is minimal, theoretical risk of increased bleeding with concomitant use. Monitor INR.
Other NSAIDs (oral)
Avoid concomitant use of topical diclofenac with oral NSAIDs due to potential for additive side effects and increased systemic exposure.
Storage
Store below 30°C. Protect from direct sunlight and heat. Do not refrigerate or freeze. Keep out of reach of children.
Overdose
Overdose with topical diclofenac is unlikely due to low systemic absorption. If excessive amount is applied, wash the skin thoroughly with water. In case of accidental ingestion, seek immediate medical attention. Symptomatic and supportive measures should be taken.
Pregnancy & Lactation
Avoid use in the third trimester of pregnancy due to potential for premature closure of the fetal ductus arteriosus. Use with caution in the first and second trimesters, only if clearly needed and under medical supervision. Consult a doctor before use during lactation; minimal systemic absorption suggests low risk, but caution is advised.

Clinical Trials
Extensively studied in numerous clinical trials establishing its efficacy and safety for localized pain and inflammation. Ongoing research may explore new formulations or specific patient populations.
Lab Monitoring
- Generally not required due to minimal systemic absorption. In cases of extensive, long-term use, systemic monitoring (e.g., renal function, liver enzymes, CBC) may be considered, but this is rare for topical forms.
Doctor Notes
- Remind patients that this is for external use only.
- Advise patients on proper application technique and hand hygiene.
- Educate on symptoms requiring discontinuation (e.g., severe skin reaction, systemic effects).
- Consider patient's history of NSAID sensitivity even for topical use.
Patient Guidelines
- For external use only. Do not ingest.
- Avoid contact with eyes, mucous membranes, and open wounds.
- Do not apply to irritated or broken skin.
- Wash hands thoroughly after application.
- Do not cover the treated area with an occlusive bandage.
- If symptoms persist for more than 7 days, consult a doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. Do not apply a double dose to make up for a forgotten one. Continue with your regular dosing schedule.
Driving Precautions
Volifen 1-V Spray is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- In conjunction with applying the spray, consider rest for the affected area.
- Applying cold packs initially for acute injuries may help reduce swelling and pain.
- For chronic pain, heat therapy might be beneficial after the acute phase.
- Maintain a healthy weight to reduce joint stress.
