Description
Overview of the medicine
Volifen-1-W Gel is a non-steroidal anti-inflammatory drug (NSAID) applied topically to relieve pain and inflammation associated with musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Apply 2 to 4 grams of gel (approx. 2.5-5 cm strip) to the affected area 3-4 times daily and rub gently into the skin.
Elderly
Same as adult dosage.
Renal_impairment
No specific dosage adjustment required for topical application, as systemic absorption is minimal.
How to Take
For external use only. Apply a small amount of gel to the affected area and gently rub it into the skin. Wash hands thoroughly after application unless the hands are the treated area.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever. When applied topically, it exerts its local anti-inflammatory and analgesic effects.
Pharmacokinetics
Onset
Pain relief usually within 30-60 minutes.
Excretion
Primarily renal (approximately 60%) and biliary/fecal (approximately 30-40%).
Half life
Approximately 1-2 hours in plasma after systemic absorption, but terminal half-life at the site of action might be longer due to tissue binding.
Absorption
Systemic absorption is low (typically <6%), but varies based on application area, skin integrity, and duration. Peak plasma concentrations are significantly lower than with oral administration.
Metabolism
Primarily hepatic via hydroxylation and glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, or other NSAIDs.
- Application to open wounds, infected skin, or eczema.
- Third trimester of pregnancy.
- Children under 12 years of age (consult physician).
Drug Interactions
Oral NSAIDs
Concomitant use with oral NSAIDs may increase the risk of systemic side effects.
Anticoagulants
Caution with anticoagulants as diclofenac may enhance their effects, although unlikely with topical application.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose with topical diclofenac gel is unlikely due to low systemic absorption. If accidentally ingested, symptomatic and supportive treatment should be given.
Pregnancy & Lactation
Use in the first and second trimesters of pregnancy only if clearly needed and after consulting a doctor. Contraindicated in the third trimester due to potential fetal harm (premature closure of ductus arteriosus). Use during lactation only if necessary, as small amounts may pass into breast milk; consult a doctor.
