Description
Overview of the medicine
Voltid SR (Diclofenac Sodium) is a non-steroidal anti-inflammatory drug (NSAID) used for symptomatic treatment of pain and inflammation in various conditions, including rheumatic disorders, osteoarthritis, and ankylosing spondylitis. The sustained release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
100 mg once daily.
Elderly
Use with caution; lower initial doses may be required.
Renal_impairment
Use with caution; dose reduction may be necessary in severe impairment. Not recommended for eGFR < 30 mL/min.
How to Take
Swallow whole with food or milk to minimize gastrointestinal upset. Do not chew or crush.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. Prostaglandins are key mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Onset of action for pain relief is typically within 1-2 hours; sustained effect for SR.
Excretion
Approximately 60% excreted in urine as metabolites, and the remainder in bile/feces.
Half life
Plasma elimination half-life is approximately 1-2 hours, but the SR formulation extends its therapeutic effect.
Absorption
Well absorbed orally; sustained release formulation ensures prolonged absorption over several hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to hydroxylated metabolites.
Side Effects
Contraindications
- •Hypersensitivity to diclofenac or other NSAIDs
- •Active gastrointestinal bleeding or ulceration
- •Severe heart failure
- •Severe renal or hepatic impairment
- •Third trimester of pregnancy
- •History of asthma, urticaria, or allergic reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increases plasma lithium levels, potentially leading to toxicity.
Warfarin
Increased risk of bleeding due to antiplatelet effect and displacement from plasma protein binding.
Diuretics
May reduce the natriuretic effect of diuretics.
ACE Inhibitors/ARBs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light.
Overdose
Symptoms include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is symptomatic and supportive; consider activated charcoal or gastric lavage within an hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C (D in 3rd trimester or near delivery). Avoid in the third trimester due to potential premature closure of ductus arteriosus. Use with caution during lactation; small amounts may pass into breast milk.
