Description
Overview of the medicine
Vomino 10 mg Tablet is an antiemetic used to treat nausea, vomiting, and certain gastrointestinal motility disorders. It helps to move food through the stomach faster and reduces symptoms of bloating and indigestion.
Uses & Indications
Dosage
Adults
10 mg, 3-4 times daily, 15-30 minutes before meals. Maximum 30 mg/day for the shortest duration possible.
Elderly
Similar to adults, but use with caution due to potential increased risk of cardiac side effects. Lowest effective dose recommended.
Renal_impairment
For severe impairment (CrCl < 30 mL/min), reduce frequency to 10 mg once or twice daily depending on severity.
How to Take
Take orally, 15-30 minutes before meals. Do not chew or crush the tablet. Swallow whole with water.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2 receptor antagonist, primarily in the chemoreceptor trigger zone (CTZ) and in the stomach. By blocking dopamine receptors, it increases gastrointestinal motility and tone, facilitating gastric emptying and preventing reflux, thereby reducing nausea and vomiting.
Pharmacokinetics
Onset
15-30 minutes for prokinetic effects.
Excretion
Approximately 31% excreted in urine and 66% in feces over 4 days, mainly as metabolites.
Half life
Approximately 7-9 hours; prolonged in severe renal impairment.
Absorption
Rapidly absorbed from the gastrointestinal tract, but low oral bioavailability (approx. 15%) due to extensive first-pass metabolism in the liver.
Metabolism
Extensively metabolized in the liver by N-dealkylation and oxidative hydroxylation, primarily via CYP3A4 enzymes.
Side Effects
Contraindications
- Known hypersensitivity to domperidone or any component of the tablet.
- Patients with prolactin-releasing pituitary tumor (prolactinoma).
- Patients with moderate or severe hepatic impairment.
- Patients with cardiac conditions like QT prolongation, significant electrolyte disturbances or underlying cardiac disease.
- Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin).
- Known complete or partial bowel obstruction or gastrointestinal hemorrhage.
Drug Interactions
Anticholinergic drugs
May antagonize the prokinetic effect of domperidone.
Dopamine agonists (e.g., bromocriptine, pergolide)
Domperidone may antagonize their effects due to its dopamine receptor blocking action.
QT-prolonging drugs (e.g., some antipsychotics, antiarrhythmics)
Increased risk of cardiac arrhythmias. Concomitant use requires caution and careful monitoring.
Potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
Increased domperidone plasma levels, significantly increasing the risk of QT prolongation and cardiac arrhythmias. Concomitant use is contraindicated.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, disorientation, extrapyramidal reactions (e.g., dystonia, parkinsonism), and convulsions. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. ECG monitoring is recommended due to QT prolongation risk.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks. Domperidone is excreted in small amounts in breast milk; therefore, use with caution during lactation. Consult a doctor.
