Description
Overview of the medicine
Vomino 5 mg suspension contains Ondansetron, an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery. It works by blocking the action of serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
For chemotherapy-induced nausea and vomiting: typically 8 mg orally 30 minutes before chemotherapy, then 8 mg every 8 hours for 1-2 days. For PONV: 16 mg orally one hour before anesthesia.
Elderly
No dosage adjustment generally required, but close monitoring is advised due to potential for prolonged half-life.
Renal_impairment
No dosage adjustment generally required.
How to Take
Oral suspension can be taken with or without food. Measure the dose accurately using the provided measuring device.
Mechanism of Action
Ondansetron is a selective serotonin 5-HT3 receptor antagonist. Serotonin 5-HT3 receptors are present both peripherally, on vagal nerve terminals, and centrally, in the chemoreceptor trigger zone (CTZ) of the area postrema. By blocking these receptors, Ondansetron prevents the activation of the vomiting reflex.
Pharmacokinetics
Onset
30 minutes to 2 hours for antiemetic effect.
Excretion
About 5% of the dose excreted unchanged in urine, remaining excreted as metabolites via urine and feces.
Half life
Approximately 3-6 hours in adults, longer in elderly and renally impaired patients.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract; undergoes first-pass metabolism.
Metabolism
Primarily hepatic via CYP enzymes (CYP1A2, CYP2D6, CYP3A4); extensive hydroxylation followed by glucuronide or sulfate conjugation.
Side Effects
Contraindications
- Known hypersensitivity to Ondansetron or any component of the formulation.
- Concomitant use with apomorphine due to risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol; risk of serotonin syndrome.
Apomorphine
Risk of profound hypotension and loss of consciousness (contraindicated).
CYP3A4 inducers (e.g., Rifampicin, Phenytoin)
May decrease plasma concentrations of ondansetron.
QT prolonging drugs (e.g., Amiodarone, Sotalol)
Increased risk of QT prolongation and Torsade de Pointes.
Storage
Store below 30°C in a dry place, away from light. Do not freeze.
Overdose
Symptoms of overdose may include transient blindness, severe constipation, hypotension, and vasovagal episodes with transient AV block. Management is supportive and symptomatic; there is no specific antidote. ECG monitoring is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Human data are limited but generally reassuring. Use only if clearly needed. Excretion in human milk is unknown; caution is advised for breastfeeding mothers.
