Description
Overview of the medicine
Vonocab 10 mg Tablet contains Vonoprazan, a potassium-competitive acid blocker (P-CAB), used to treat acid-related disorders like gastric ulcers, duodenal ulcers, and reflux esophagitis. It works by suppressing gastric acid secretion.
Uses & Indications
Dosage
Adults
For gastric ulcer, duodenal ulcer, reflux esophagitis: 10-20 mg once daily. For H. pylori eradication: 20 mg twice daily in combination with antibiotics.
Elderly
No specific dosage adjustment is generally required, but use with caution in elderly patients with impaired hepatic or renal function.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take the tablet orally, with or without food. It is often recommended to take it in the morning for optimal effect.
Mechanism of Action
Vonoprazan is a P-CAB that reversibly inhibits the H+/K+-ATPase enzyme in gastric parietal cells by competing with potassium ions. This leads to a potent and sustained suppression of gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression effect typically observed within 1 hour.
Excretion
Excreted primarily through urine (approximately 60-70%) and feces (approximately 20-30%).
Half life
Elimination half-life is approximately 7-9 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Cmax) is reached within 1.5-2 hours. Absolute bioavailability is approximately 80%.
Metabolism
Primarily metabolized by CYP3A4, with minor contributions from CYP2B6 and CYP2D6.
Side Effects
Contraindications
- Known hypersensitivity to Vonoprazan or any component of the formulation.
Drug Interactions
CYP3A4 Inhibitors
Co-administration with potent CYP3A4 inhibitors (e.g., clarithromycin, ketoconazole) may increase plasma concentrations of Vonoprazan.
Drugs requiring acidic gastric pH
Vonoprazan significantly suppresses gastric acid secretion, which may alter the absorption of drugs that depend on an acidic gastric pH for bioavailability (e.g., atazanavir, nelfinavir, iron salts, some antifungals like ketoconazole).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for Vonoprazan overdose. In case of an overdose, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk. Exercise caution when administered to a nursing woman. Consult a doctor.

Clinical Trials
Extensive clinical trials have demonstrated Vonoprazan's superior or non-inferior efficacy compared to traditional PPIs for various acid-related disorders and H. pylori eradication.
Lab Monitoring
- No routine lab monitoring is generally required for Vonoprazan. However, liver function tests may be considered in patients with pre-existing hepatic impairment.
Doctor Notes
- Vonoprazan offers rapid and potent acid suppression, potentially beneficial for PPI non-responders.
- Consider this P-CAB for patients with severe reflux esophagitis or those requiring higher pH maintenance.
- Monitor for potential drug interactions, especially with pH-dependent medications.
Patient Guidelines
- Take Vonocab exactly as prescribed by your doctor.
- Do not stop taking the medicine abruptly without consulting your doctor.
- Report any unusual side effects or worsening of symptoms to your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Vonoprazan may cause dizziness or visual disturbances in some patients. If you experience these effects, avoid driving or operating machinery.
Lifestyle Advice
- Avoid spicy, fatty foods, alcohol, and caffeine, especially if you have acid reflux.
- Quit smoking as it can worsen acid-related symptoms.
- Eat smaller, more frequent meals and avoid lying down immediately after eating.
