Description
Overview of the medicine
Vonocab 20 mg tablet contains Vonoprazan, a potassium-competitive acid blocker (PCAB) used to treat acid-related disorders of the stomach and esophagus by suppressing gastric acid secretion.
Uses & Indications
Dosage
Adults
Gastric/Duodenal Ulcer: 20 mg once daily for 6-8 weeks. Reflux Esophagitis: 20 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 7 days in combination with antibiotics.
Elderly
No specific dosage adjustment required based on age, but caution is advised due to potential comorbidities.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take the tablet orally, preferably in the morning, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Vonoprazan reversibly inhibits the H+, K+-ATPase enzyme (proton pump) in the gastric parietal cells by competing with potassium ions. This leads to a potent and prolonged suppression of gastric acid secretion.
Pharmacokinetics
Onset
Within hours of administration, acid suppression is achieved.
Excretion
Mainly via urine and feces.
Half life
Approximately 7-9 hours (terminal elimination half-life).
Absorption
Rapidly absorbed, reaching peak plasma concentrations within 1.5 to 2 hours.
Metabolism
Primarily metabolized in the liver by CYP3A4 and to a lesser extent by CYP2B6, CYP2C19, CYP2D6.
Side Effects
Contraindications
- Hypersensitivity to Vonoprazan or any component of the formulation
- Concomitant use with Atazanavir, Nelfinavir, or Rilpivirine (due to reduced absorption of antivirals)
Drug Interactions
Digoxin
Increased absorption of digoxin, requiring monitoring of digoxin levels.
Warfarin
Monitor INR, as vonoprazan may affect warfarin metabolism.
Methotrexate
Possible increase in methotrexate exposure.
Atazanavir, Nelfinavir, Rilpivirine
Reduced absorption of these antiviral drugs, leading to decreased efficacy. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Treatment should be symptomatic and supportive. Vonoprazan is not effectively removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown if Vonoprazan is excreted in human milk; caution should be exercised when administered to a nursing mother.
