Description
Overview of the medicine
Vonopa 20 mg tablet contains Vonoprazan, a potassium-competitive acid blocker (P-CAB) used to treat acid-related disorders like gastric ulcers, duodenal ulcers, reflux esophagitis, and for H. pylori eradication. It provides rapid and sustained acid suppression.
Uses & Indications
Dosage
Adults
For gastric ulcer, duodenal ulcer, reflux esophagitis: 20 mg once daily. For H. pylori eradication: 20 mg twice daily with appropriate antibiotics.
Elderly
No specific dosage adjustment needed unless severe hepatic or renal impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Take orally, once daily, with or without food. For H. pylori eradication, take as directed with antibiotics.
Mechanism of Action
Vonoprazan is a potassium-competitive acid blocker (P-CAB) that reversibly inhibits the H+/K+-ATPase proton pump in gastric parietal cells, thereby suppressing gastric acid secretion. Unlike PPIs, it does not require acid activation and binds to the pump independently of its activation state.
Pharmacokinetics
Onset
Within hours for significant acid suppression.
Excretion
Excreted predominantly in urine (major) and feces (minor).
Half life
Approximately 7-9 hours (terminal elimination half-life).
Absorption
Rapidly absorbed after oral administration, peak plasma concentration (Cmax) reached within 1.5-2 hours. Bioavailability is high.
Metabolism
Primarily metabolized in the liver mainly by CYP3A4, with minor contributions from CYP2B6, CYP2C19, and CYP2D6.
Side Effects
Contraindications
- Hypersensitivity to Vonoprazan or any component of the formulation.
- Co-administration with atazanavir or nelfinavir (for HIV treatment).
Drug Interactions
Clopidogrel
Potential for reduced antiplatelet effect, though less significant than with some PPIs.
Methotrexate
Increased plasma levels of methotrexate, especially with high doses.
Drugs dependent on gastric pH
Vonoprazan reduces gastric acid, potentially affecting absorption of drugs like ketoconazole, itraconazole, atazanavir, nelfinavir, iron salts.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for Vonoprazan overdose. In case of overdose, symptomatic and supportive treatment should be administered. Hemodialysis is unlikely to be effective due to high protein binding.
Pregnancy & Lactation
Pregnancy Category B. Animal studies show no evidence of harm to the fetus, but human data are limited. Use only if clearly needed. It is not known whether vonoprazan is excreted in human milk, use with caution during lactation.
