Description
Overview of the medicine
Vortioxetine is an antidepressant used for the treatment of major depressive disorder (MDD) in adults. It works by affecting multiple serotonin receptors and inhibiting serotonin reuptake in the brain.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily. May be increased to 10 mg once daily, then up to a maximum of 20 mg once daily based on patient response and tolerability.
Elderly
Generally similar to adult dosage, but caution advised. Initial dose 5 mg once daily, with careful titration.
Renal_impairment
No dose adjustment is required for mild to moderate renal impairment. Data for severe renal impairment is limited, but generally no dose adjustment is expected.
How to Take
Vortioxetine tablets should be taken orally once daily, with or without food.
Mechanism of Action
Vortioxetine acts as a serotonin reuptake inhibitor (SRI), an agonist at 5-HT1A receptors, a partial agonist at 5-HT1B receptors, and an antagonist at 5-HT3, 5-HT1D, and 5-HT7 receptors. This multimodal action is believed to contribute to its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Clinical antidepressant effects may be observed within 1-2 weeks, with full effect potentially taking several weeks.
Excretion
Primarily excreted in urine (approx. 59%) and feces (approx. 26%) as inactive metabolites.
Half life
Approximately 66 hours (range 57-73 hours), allowing for once-daily dosing.
Absorption
Well absorbed after oral administration, peak plasma concentration usually reached within 7-11 hours. Bioavailability is approximately 75%.
Metabolism
Primarily metabolized in the liver via CYP2D6, CYP3A4/5, CYP2C9, CYP2C19, CYP2A6, CYP2B6, and CYP2C8.
Side Effects
Contraindications
- Hypersensitivity to vortioxetine or any excipients
- Concomitant use with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- Concomitant use with linezolid or intravenous methylene blue
Drug Interactions
MAOIs
Increased risk of serotonin syndrome.
Warfarin
Potential for altered anticoagulant effects, monitor INR.
Serotonergic drugs
Increased risk of serotonin syndrome (e.g., SSRIs, SNRIs, triptans).
Strong CYP inducers
Consider dose increase of vortioxetine (e.g., rifampicin, carbamazepine, phenytoin).
Strong CYP2D6 inhibitors
Consider dose reduction of vortioxetine (e.g., bupropion, fluoxetine, paroxetine).
Storage
Store at room temperature (20-25°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, dizziness, abdominal discomfort, diarrhea, generalized pruritus, somnolence. Management is primarily supportive, involving monitoring of vital signs and cardiac rhythm, and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk. Discuss with a doctor.

Clinical Trials
Multiple randomized, double-blind, placebo-controlled trials have demonstrated the efficacy and safety of vortioxetine in the treatment of MDD. Studies have also explored its effects on cognitive function in MDD patients.
Lab Monitoring
- Monitor for hyponatremia, especially in elderly patients or those taking diuretics.
- Monitor for signs and symptoms of serotonin syndrome.
Doctor Notes
- Counsel patients on potential delayed onset of action and importance of adherence.
- Monitor for suicidal ideation, especially at treatment initiation or dose changes.
- Consider CYP2D6 status for dose adjustment if needed.
- Warn about potential for angle-closure glaucoma in susceptible individuals.
Patient Guidelines
- Take as directed by your doctor.
- Do not stop taking abruptly without consulting your doctor, as withdrawal symptoms may occur.
- Report any unusual mood changes or suicidal thoughts immediately to your doctor.
- Avoid alcohol while on this medication.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
May cause dizziness, disturbed vision, or impaired concentration. Patients should be cautioned about operating hazardous machinery, including automobiles, until they are reasonably certain that vortioxetine therapy does not adversely affect their ability to engage in such activities.
Lifestyle Advice
- Engage in regular physical activity.
- Maintain a balanced diet and adequate sleep.
- Practice stress-reduction techniques.
