Overview of the medicine
Wedica 50 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat various mental health conditions by helping to restore the balance of a certain natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose is typically 25-50 mg once daily. May be increased gradually to a maximum of 200 mg daily, depending on response and tolerability.
Elderly
Similar to adult dosage, but often started at a lower dose (e.g., 25 mg) and increased more slowly due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment is generally required in patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally, once daily, in the morning or evening, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced and prolonged serotonergic neurotransmission.

Onset
Therapeutic effects may take 1-4 weeks to become apparent, with full benefits seen after several weeks of consistent use.
Excretion
Excreted in both urine (40-45%) and feces (40-45%) as metabolites; less than 2% is excreted unchanged in urine.
Half life
Approximately 26 hours for sertraline and 62-104 hours for its active metabolite, N-desmethylsertraline.
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring 4.5-8.4 hours after administration. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2B6, CYP2C9, CYP3A4, and CYP2D6, to an active metabolite (N-desmethylsertraline) and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to sertraline or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI (or initiating an MAOI within 14 days of discontinuing sertraline).
- Concomitant use with pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome (fever, rigidity, mental status changes).
Alcohol
Enhances CNS depressant effect; avoid concomitant use.
Pimozide
Increased pimozide levels leading to QTc prolongation.
Warfarin
Increased prothrombin time, monitor INR closely.
NSAIDs, Aspirin
Increased risk of bleeding.
Other Serotonergic Drugs (e.g., Triptans, Tricyclic Antidepressants, Fentanyl, Lithium, Tramadol, St. John's Wort)
Increased risk of serotonin syndrome.

Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and coma. Management is supportive and symptomatic, maintaining airway, breathing, and circulation. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Sertraline is excreted in breast milk; caution is advised when administered to a nursing mother.
