Overview of the medicine
Winfax 250 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against various gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg daily, given as a single dose or in two divided doses of 200 mg every 12 hours. For 250 mg tablets, it can be 250 mg twice daily. Duration of treatment varies from 7 to 14 days depending on the infection.
Elderly
No dosage adjustment is generally required for elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is recommended for patients with creatinine clearance less than 60 mL/min. For CrCl 21-60 mL/min, 300 mg daily. For CrCl <20 mL/min, 200 mg daily.
How to Take
Winfax tablets can be taken orally with or without food. It is recommended to take it at approximately the same time each day to maintain consistent drug levels.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This binding prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 2-6 hours after oral administration.
Excretion
Excreted primarily by the kidneys, with about 50% of the absorbed dose excreted unchanged in the urine within 24 hours. Small amounts are also excreted in bile and feces.
Half life
The plasma half-life is typically 3-4 hours in individuals with normal renal function.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly alter the extent of absorption.
Metabolism
Cefixime is not extensively metabolized in the body. Little or no active metabolites have been identified.
Side Effects
Contraindications
- Patients with known hypersensitivity to Cefixime, other cephalosporins, or to any of the excipients.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
Concurrent use may increase Cefixime Cmax and AUC.
Carbamazepine
Cefixime may increase carbamazepine levels when co-administered.
Warfarin and Anticoagulants
May increase prothrombin time (PT) with or without clinical bleeding, requiring careful monitoring.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by dialysis. Treatment is generally symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.
