Overview of the medicine
X-Cite 50 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat various mood and anxiety disorders. It helps improve mood, sleep, appetite, and energy levels, and decreases panic attacks and fear.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mg once daily, gradually increased to a maximum of 200 mg daily. Dosage should be individualized based on response and tolerability.
Elderly
Similar to adult dosage, but with careful monitoring due to potential for decreased clearance and increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, in the morning or evening, with or without food. Swallow the tablet whole; do not crush, chew, or break.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) in the brain, leading to increased serotonin concentrations in the synaptic cleft. This potentiation of serotonergic activity is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Therapeutic effects may take 2-4 weeks, sometimes longer, to become evident.
Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, primarily as metabolites. Less than 0.2% excreted unchanged in urine.
Half life
Approximately 26 hours (range 22-36 hours) for sertraline, and 62-104 hours for its active metabolite, N-desmethylsertraline.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached within 4.5-8.4 hours. Food increases AUC and Cmax slightly.
Metabolism
Extensive first-pass metabolism in the liver by multiple CYP isoforms (primarily CYP2C19, CYP2B6, CYP2C9, CYP3A4, CYP2D6).
Side Effects
Contraindications
- •Hypersensitivity to sertraline or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- •Concomitant use with pimozide.
Drug Interactions
MAOIs
Risk of serious, sometimes fatal, reactions including serotonin syndrome.
Lithium
Increased tremor and neurotoxic effects.
Pimozide
Increased pimozide levels and cardiotoxicity.
Triptans
Risk of serotonin syndrome.
Warfarin
Increased prothrombin time and bleeding risk.
NSAIDs/Aspirin
Increased risk of bleeding, particularly gastrointestinal.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, nausea, dizziness, agitation, and tremor. Management is primarily supportive, maintaining an open airway, monitoring cardiac and vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Sertraline is excreted in breast milk; advise caution during lactation.
