Description
Overview of the medicine
Xenap (Esomeprazole) is a proton pump inhibitor that reduces stomach acid production, used to treat conditions like GERD, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on condition and severity.
Elderly
No specific dose adjustment needed based on age alone, but monitor for concomitant conditions.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe impairment, use with caution.
How to Take
Take whole with water, at least one hour before a meal. Do not chew or crush. For patients with swallowing difficulties, tablets can be dispersed in non-carbonated water.
Mechanism of Action
Esomeprazole inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, blocking the final step of acid secretion, thereby reducing gastric acidity.

Mainly via urine (about 80%) and feces (about 20%).
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration, bioavailability is about 64% (for 20 mg) and 89% (for 40 mg) after single dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole or any benzimidazole substituted PPIs
- Concomitant use with nelfinavir
Drug Interactions
Diazepam
Decreased clearance of diazepam.
Warfarin
Increased INR/bleeding risk. Monitor INR closely.
Clopidogrel
Reduced antiplatelet effect, avoid concomitant use if possible.
Methotrexate
Increased methotrexate levels. Consider dose reduction or temporary discontinuation of PPI.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach and sight of children.
Overdose
Symptoms are generally mild and include gastrointestinal disturbances like nausea, vomiting, abdominal pain, and diarrhea. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Excretion into breast milk is unknown; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
