Overview of the medicine
Xerosec 20 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid and treat conditions like ulcers and GERD.
Uses & Indications
Dosage
Adults
Duodenal ulcer: 20 mg once daily for 2-4 weeks. Gastric ulcer/GERD: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial dose 60 mg once daily.
Elderly
No specific dosage adjustment is usually required, but use with caution in severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take one capsule orally, preferably in the morning before food. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly inhibiting the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1 hour, maximal effect within 2 hours.
Excretion
Approximately 80% of metabolites are excreted in the urine and 20% in the feces.
Half life
Plasma elimination half-life is 0.5-1 hour, but duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 0.5-3.5 hours. Bioavailability is about 30-40%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with Nelfinavir.
Drug Interactions
Warfarin
Omeprazole may prolong the elimination of warfarin, leading to increased INR and bleeding risk. Close monitoring is recommended.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel. Concomitant use should be avoided.
Methotrexate
Concomitant use with methotrexate may increase methotrexate plasma concentrations and prolong its elimination, potentially leading to toxicity.
