Overview of the medicine
Xerosec 40 mg capsule contains Esomeprazole, a proton pump inhibitor that reduces acid production in the stomach, used to treat conditions like GERD, ulcers, and H. pylori infection.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 40 mg once daily for 10 days (with antibiotics). Zollinger-Ellison Syndrome: 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment required for elderly patients, but caution in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with water at least 30-60 minutes before a meal, preferably in the morning. Do not chew, crush, or break the capsule.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion.

Onset
Within 1 hour (acid suppression).
Excretion
Metabolites are primarily excreted in the urine (approximately 80%) and feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 50-64% (for 40 mg dose), increasing with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an HIV protease inhibitor).
Drug Interactions
Digoxin
May increase digoxin levels.
Warfarin
May increase INR and prothrombin time, increasing risk of bleeding.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Atazanavir, Nelfinavir
May significantly reduce plasma concentrations, leading to loss of virologic response.
Ketoconazole, Itraconazole
May reduce absorption due to increased gastric pH.
