Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Not typically used for adults in this strength/form; higher strengths (e.g., 200 mg or 400 mg tablets) are preferred for adults.
Elderly
No specific dose adjustment necessary for elderly patients with normal renal function.
Children
8 mg/kg/day, given as a single daily dose or in two equally divided doses. For children weighing over 50 kg or older than 12 years, the recommended adult dose applies.
Renal_impairment
Dose adjustment is required. For creatinine clearance 21-60 mL/min, reduce daily dose by 50%. For creatinine clearance < 20 mL/min, reduce daily dose by 75%.
How to Take
Oral administration. The suspension should be shaken well before use. Can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to cell lysis and death.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Mainly through urine (renal excretion) and bile
Half life
Approximately 3-4 hours
Absorption
Approximately 40-50% orally absorbed, with peak plasma concentrations occurring 2-8 hours after administration.
Metabolism
Partially metabolized in the liver, but primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotic.
- •Patients with a history of severe hypersensitivity to penicillin (e.g., anaphylaxis).
Drug Interactions
Nifedipine
May increase cefixime's bioavailability.
Carbamazepine
Concomitant use may increase carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time and international normalized ratio (INR), leading to an increased risk of bleeding. Close monitoring is recommended.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep the reconstituted suspension in a refrigerator (2-8°C) and discard any unused portion after 7 days. Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Management should be supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Caution should be exercised when administered to a nursing woman.
