Xifim 200 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including those affecting the urinary tract, ears, throat, lungs, and skin. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 200-400 mg daily, given as a single dose or in two divided doses (e.g., 200 mg every 12 hours). For uncomplicated gonorrhea, a single oral dose of 400 mg is recommended.
Elderly
No dose adjustment is typically required based solely on age, but renal function should be assessed, and dosage adjusted accordingly if renal impairment is present.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 75% of the standard dose. For creatinine clearance <20 mL/min, the dose should be reduced to 50% of the standard dose.
How to Take
Take Xifim 200 mg Capsule orally with a glass of water, with or without food. It is recommended to take it at regular intervals as prescribed by your doctor. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This binding prevents the cross-linking of peptidoglycan chains, which is crucial for bacterial cell wall strength and integrity, leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Approximately 50-60% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion occurs via bile/feces.
Half life
Plasma half-life is typically 3-4 hours, potentially longer in elderly or renally impaired patients.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly affected by food.
Metabolism
Cefixime is not extensively metabolized; about 10% of the dose may be converted to an inactive metabolite.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- •History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase the absorption and peak serum levels of cefixime.
Probenecid
May increase Cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time (PT) with or without bleeding. Close monitoring of PT/INR is recommended.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B: Studies in animals have not shown a risk, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Lactation: Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.
