Description
Overview of the medicine
Ximeclav 500 mg Tablet is an oral antibiotic combining a third-generation cephalosporin (Cefixime) with a beta-lactamase inhibitor (Clavulanic Acid). It is used to treat a wide range of bacterial infections by killing the bacteria and preventing their resistance.
Uses & Indications
Dosage
Adults
One Ximeclav 500 mg tablet (Cefixime 500 mg + Clavulanic Acid 125 mg) once or twice daily, depending on the severity of the infection. Duration typically 5-10 days, or as directed by physician.
Elderly
No specific dose adjustment is generally required unless severe renal impairment is present.
Renal_impairment
For patients with creatinine clearance < 30 mL/min, the dose should be reduced and/or the dosing interval extended. Consult physician for specific guidance.
How to Take
Take orally with a full glass of water. It can be taken with or without food. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. Clavulanic Acid is a beta-lactamase inhibitor that protects cefixime from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending its antibacterial spectrum.
Pharmacokinetics
Onset
Antibacterial effect onset is typically within a few hours of administration.
Excretion
Cefixime is primarily excreted unchanged in the urine (about 50%). Clavulanic acid is also largely excreted by the kidneys.
Half life
Cefixime: approximately 3-4 hours. Clavulanic Acid: approximately 1 hour.
Absorption
Cefixime is approximately 40-50% absorbed orally. Clavulanic acid is rapidly and well absorbed after oral administration.
Metabolism
Cefixime is not extensively metabolized. Clavulanic acid undergoes extensive metabolism.
Side Effects
Contraindications
- Known hypersensitivity to cefixime, clavulanic acid, other cephalosporins, or penicillins.
- History of severe allergic reactions to any beta-lactam antibiotic.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing its renal tubular secretion.
Live Typhoid Vaccine
Antibiotics may reduce the effectiveness of live bacterial vaccines. Administer at least 3 days apart.
Oral Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, leading to increased INR and risk of bleeding. Close monitoring is recommended.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include increased gastrointestinal side effects (e.g., nausea, vomiting, diarrhea). Management is primarily symptomatic and supportive; gastric lavage may be considered. No specific antidote is available.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy; only if clearly needed and the potential benefits outweigh the risks. Small amounts of cefixime are excreted in breast milk; caution is advised during breastfeeding.

Clinical Trials
Cefixime and Clavulanic Acid, both individually and in combination, have undergone extensive clinical trials establishing their efficacy and safety profiles for various bacterial infections.
Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) for prolonged therapy or in patients with renal impairment.
- Liver function tests (e.g., ALT, AST) if hepatic dysfunction is suspected.
- Complete Blood Count (CBC) for patients on prolonged therapy.
Doctor Notes
- Prescribe based on susceptibility testing where possible, especially in recurrent or severe infections.
- Emphasize patient compliance to complete the full course of treatment to prevent therapeutic failure and resistance development.
- Monitor for signs of hypersensitivity reactions, C. difficile infection, and changes in renal/hepatic function during extended therapy.
Patient Guidelines
- Take the medicine for the full prescribed duration, even if symptoms improve, to prevent recurrence and antibiotic resistance.
- Do not share this medication with others, as it may not be suitable for their condition.
- Report any severe or persistent side effects to your doctor immediately.
- Store the medicine as directed.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
