Ximeclav 250 mg Tablet is a combination antibiotic containing Cefuroxime axetil and Clavulanic acid. It is used to treat a wide range of bacterial infections susceptible to this combination, including respiratory tract infections, urinary tract infections, and skin and soft tissue infections.
Uses & Indications
Dosage
Adults
One 250 mg/62.5 mg tablet twice daily for 5-10 days, depending on the severity and type of infection. For more severe infections, a higher strength (e.g., 500 mg/125 mg) may be indicated.
Elderly
No specific dose adjustment is required in elderly patients with normal renal function. Dosage should be adjusted based on renal function if impaired.
Renal_impairment
Dosage adjustment is necessary in patients with significant renal impairment. Consult product leaflet for specific creatinine clearance guidelines.
How to Take
Take orally, preferably with food, as food enhances the absorption of cefuroxime axetil. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that prevents the breakdown of cefuroxime by beta-lactamase enzymes produced by some bacteria, thereby extending its antibacterial spectrum.
Pharmacokinetics
Onset
Rapid, with peak plasma concentrations typically achieved within 1-2 hours after oral administration.
Excretion
Both cefuroxime and clavulanic acid are primarily excreted by the kidneys, with cefuroxime being excreted unchanged via glomerular filtration and tubular secretion, and clavulanic acid as metabolites and unchanged drug.
Half life
Cefuroxime: approximately 1.2-1.5 hours. Clavulanic Acid: approximately 1 hour.
Absorption
Cefuroxime axetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Clavulanic acid is also well absorbed. Absorption is enhanced when taken with food.
Metabolism
Cefuroxime is not extensively metabolized. Clavulanic acid is extensively metabolized to inactive compounds.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, clavulanic acid, or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylaxis) to any other beta-lactam antibiotic (e.g., penicillins).
- History of cholestatic jaundice/hepatic dysfunction associated with penicillin or cefuroxime/clavulanic acid therapy.
Drug Interactions
Probenecid
Concomitant use with probenecid increases cefuroxime plasma concentrations and prolongs its half-life by reducing renal tubular secretion.
Oral Anticoagulants
May prolong prothrombin time (INR) in patients receiving oral anticoagulants. Close monitoring is advised.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; additional non-hormonal contraception may be advisable.
Antacids/H2 blockers
May decrease the bioavailability of cefuroxime axetil; administer at least 1-2 hours apart.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal disturbances (nausea, vomiting, diarrhea) and convulsions. Management is supportive, and hemodialysis may be considered to remove the drug from circulation.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy and lactation only if clearly needed and under strict medical supervision. Small amounts of cefuroxime and clavulanic acid are excreted in breast milk.
