Description
Overview of the medicine
Ximocef 200 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic. It is effective against a wide range of bacterial infections including those of the respiratory tract, urinary tract, ear, nose, throat, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
200-400 mg once daily or 200 mg every 12 hours, depending on the severity of infection. Duration usually 7-14 days.
Elderly
No dosage adjustment needed unless severe renal impairment.
Renal_impairment
Dose adjustment required for creatinine clearance <60 mL/min. For CrCl 21-60 mL/min, 200 mg daily; for CrCl <20 mL/min, 100 mg daily.
How to Take
Ximocef 200 mg Capsule should be taken orally, with or without food. For best absorption and to minimize gastrointestinal upset, it is often recommended to take it with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, ultimately leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Typically within 1 hour.
Excretion
Mainly via renal excretion (approximately 50% unchanged in urine) and biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 30-50% orally absorbed. Food slightly delays absorption but does not significantly affect the extent.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporins
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Carbamazepine
Cefixime may increase carbamazepine levels.
Oral contraceptives
May reduce efficacy of oral contraceptives (though controversial, advisable to use additional contraception).
Warfarin and other anticoagulants
May increase prothrombin time (INR), requiring close monitoring and dose adjustment of anticoagulants.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Small amounts are excreted in breast milk; use with caution in breastfeeding mothers.
