Description
Overview of the medicine
Xitabin 500 mg Tablet is a broad-spectrum antibiotic belonging to the fluoroquinolone class. It is effective against a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily or 250 mg twice daily, depending on the severity and type of infection. Duration usually 7-14 days.
Elderly
No specific dose adjustment is generally required unless there is significant renal impairment. Monitor renal function.
Renal_impairment
Dosage reduction may be necessary for patients with creatinine clearance < 50 mL/min. Consult physician for specific guidelines.
How to Take
Take the tablet whole with water, with or without food. Do not crush or chew. It is advisable to take at the same time each day.
Mechanism of Action
Xitabine inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Bactericidal effects typically begin within 1-2 hours of administration.
Excretion
Mainly excreted by the kidneys, with a significant portion appearing unchanged in urine.
Half life
Approximately 6-8 hours, allowing for once or twice daily dosing.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily metabolized in the liver, forming inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Xitabine or other fluoroquinolones.
- History of tendon disorders related to fluoroquinolone administration.
- Children and adolescents below 18 years of age (due to potential for cartilage damage).
Drug Interactions
Warfarin
Enhanced anticoagulant effect, leading to increased bleeding risk. Monitor INR closely.
Theophylline
Increased plasma levels of theophylline, potentially leading to toxicity. Monitor theophylline levels.
Antacids, iron, zinc, sucralfate
Reduced absorption of Xitabine. Administer Xitabine at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage and activated charcoal may be considered. Monitor vital signs and ECG.
Pregnancy & Lactation
Not recommended during pregnancy and lactation due to potential risks to the fetus/infant. Consult a doctor to weigh benefits against risks.
